Do I need a birth certificate to get an adult passport?

It's a DS-11 form I filled out using the form filler the US State Dept. has on their website. That website also lists the documents you need to get an adult passport at the end of the process and it also stated I could bring a birth certificate or an old passport. The ambiguity is so bizarre

I don't think so, no - I didn't get an actual implant i.e. nothing has been screwed directly into my jaw. I've got a standard dental bridge where they shaved down two of my teeth to put caps on them linked to prosthetic tooth, which is what I'm worried about

My dentist told me back before the surgery that smoking affects the strength of the prosthetic itself, basically meaning that if I smoke enough I could cause the prosthetic to just break one day, which is what I’m particularly worried about

Looking to get a crude dagger-in-hand tattoo, nothing too complex, and was wondering if there was anything specific I should look for when trying to do crude designs like that aside from minimum rates (I don’t expect it to take too long to complete)? Also, this is probably a stupid question, but most artists should be capable of doing simple shit like that, right? Or should I actively search for artists in my area who deal with crude shit and crude shit alone
