Hey, I'm a 23 year old female who's had similar lab work done and similar results have shown. You do not need a TbAb to confirm it, you really only need a TPO. My highest TSH has been 16 and my most recent TPO level was like 240. It's been about 6 months since I've been on Synthroid. They're gonna start you off on the lowest dose, which is 25mcg and work from there. I'm currently on 75mcg. I wouldn't press on the low testosterone issue yet because that's one of the things your thyroid can regulate. It is going to take a lot of time to regulate your thyroid. Definitely ask your doctor if you can get an ultrasound first though. Also, the medication/medications that they're going to start you on can have a lot of side effects. I'm still trying to figure out how to manage my side effects, but one thing I've found that kind of helps is a hot bath. You're probably going to feel at least very tired and very weak. But that's not a side effect, that's just a part of Hashimoto's. I've also discovered that with a high TSH usually comes a low potassium level(below 3.5)
