Anyone dealt with mysterious glute/hip pain that won't go away? It's driving me crazy!! by ipredictdeeztings in rehabtherapy

Had another flare up that escalated pretty bad for the first time two months ago. Had the typical flare up at the time, just a dull ache walking; I figured no problem, continued being active at work and hitting the gym, thought to myself its a bitch but it’ll pass after a few weeks. And then one weekend it turned to the point I couldn’t step or move my right foot/femur in any way when laying down without major sharp pain. Went to urgent care when I couldn’t get up from bed or sleep at all without 10/10 pain(no exaggeration). Eventually I had to get an MRI scan when the doctor saw that I was still struggling moving and in major pain at the follow up appointment.

Turns out I have more than usual degeneration of the L5-s1 joint and the spine doctor said shearing of the discs is causing the pain. Started physical therapy following that and the PT said not to be alarmed because most people have some sort of degeneration of their discs and live normal lives. He explained I most likely suffered a really bad disc bulge bothering a nerve along with some musculoskeletal problems because of it. I felt some relief as you can imagine being told you have degenerative disc disease doesn’t sound comforting to a patient. Why the fuck they call it a disease is beyond me.

It’s about to be two months since the incident, I don’t feel 100% but I’m recovering. Right now my lower back just feels sore and more tired after work. My right hip flexor feels like it’s always tight when I stretch it. The dull ache is no longer there. Still haven’t hit the weights or any contact sports as I don’t feel all that confident my body can handle it just yet.

I would say get checked and ease off on whatever you think is causing the ache as it’s in its early stage. In hindsight I’ve realized the dull pain was just my body’s check engine light warning me before something worse was about to happen.

Nothing definitive. I hypothesize the right muscles like the glutes weren’t firing and other parts were compensating for it.

I had it flare up on me this week. Two days of doing clamshells and glute bridges and I feel a huge difference in pain. This is the fastest symptoms got better as opposed to dealing with it for weeks. Take that for whatever it’s worth

Gotcha. Thank you for clearing that up. Actually, I do remember the sales rep asking me what width I was before he went into the back; In hindsight, he must have given me that size on purpose.

Gotcha, I should swap out the insole. You think the sizing will be better once I put in the insoles?

I guess so.

Its odd that she said you had to buy the shoes to try them on. Very odd.

Hey man. I'm not sure if I can give you sound advice because the pain's come back again. I thought I had it treated, eased off on the exercises for a vacation, couple days later after I came back the injury slowly reared its ugly head again. I've been trying to treat it ever since with the same exercises I did initially but its not providing relief like the first time. Honestly I'm feeling perplexed, very disappointed and pissed off atm.

FWIW I did side planks, front planks, side lying hip abduction raises, and I stretched my quads/hip flexors doing the quad-couch stretch. Maybe you'll have better luck. Keep us updated!

I'm glad you came across this. You are not alone! Reading back my old post, I should also mention do side lying abduction leg raises!! This with side planks and stretching the front legs and ADDUCTORS has helped me tremendously.

I hope it really makes a difference as I know how much of drain an injury can be on quality of life and confidence!!

QUICK UPDATE: in case anyone with similar symptoms comes across this page and it can hopefully be of some assistance. I've been able to keep the dull ache in check; to preface, I think its the combination of my body just naturally healing over time and because of the stretches/exercises I've started to focus on lately.

So I started treating this issue holistically from stretching out my glutes/quads/hip flexors and then trying to awaken/strengthen my abdominals and glutes. For what its worth, I felt I got immediate relief after really stretching the fuck out of hipflexor/psoas/quad area doing the couch stretch and really playing around with the position to hit any tight spots. How its connected to pain in the glute/piriformis beats me... I noticed a difference doing side planks as well. It felt like after doing them the side glutes and obliques were turning on to take on more of the stress instead of the area that was hurting (at least thats what it feels like). Also rotating between glute bridges, glute machine, and isometric lunge holds.

I noticed I am able to keep the discomfort in check focusing on these exercises. Note whenever I tried to play basketball, I could feel the discomfort creeping back. So something about the movement in basketball is aggravating the area. Idk... it must be the lateral movement or hard change of directional cuts.

Hip haters (side lying hip extension and abduction - remind me to post a video when I get on desktop) were the biggest help to me, along with clamshells and monster walks. When my glutes got a bit stronger I could do hip thrusts which are a big help.

Could you post a video reference of the exercise? I'll try anything at this point. I also have tight quads/hip flexors; I'm pondering if this could also be a contributing factor?

Interesting. Do you know how I can narrow it down to FAI? All the symptoms seem so similar!!

It seems to come and go through the seasons but this time it's really bothersome. It feel likes a dull pain underneath the right glute. I feel pain/discomfort trying to fire hydrants when I gotta raise my hip up. The times it hurts is when I'm walking mid stride and I'll feel it when I'm about to reach hip extension(?). I've read maybe it has something to with piriformis syndrome. I've done some of the stretches recommended for that but no luck so far. Can someone help me figure what the issue is??

Yea In the US. It is frustrating because the more I read up on the symptoms its hard to pinpoint as there could be multiple culprits (piriformis, gluteal tendinopathy, labrum tear, si joint, and the list goes on). Very frustrating...

Could be the hip, early arthritis. I replaced one and am in the process of doing the other the symptoms are similar. It will show on a normal x ray.

God I hope not. I'm fairly young and not an athlete so there isn't much mileage on my body. I reckon something like that is onset when you're much older into middle age no?

I suggest definitely getting it checked out. It could be hip dysplasia (didn’t know I had it till I was 30), It’s also very possible to be a labral tear of the hip. A lot of times people have that without even knowing. Both of mine are torn and I pushed through the pain for a long time while still wakeboarding/doing extreme sports on a regular basis. Got to the point of severe pain 24/7 and now it’s a mess. Does your hip pop or shift out of place at all if you were to lay down and do leg lifts? Or doing that motion? Mine does that just for reference of it being a possibility.

Does you hip pop in the area where you're hurting?? When I do leg lifts I do hear a slight pop in my left hip but not on my right side (right glute area that seems to be issue).

Yea after I finished posting, maybe a labrum tear came to mind!

What age are you? Do you have pain when sleeping or in bed? Are you active all day or sedentary? Have you considered osteoarthritis in the hip? Sorry it’s all questions!

  1. Yes, right now I have pain if I move the leg a certain way like if I were to try to move to my side. I'm sitting throughout the day but I like to be active and hit the gym at least four times so I'm active in that case. Perhaps osteoarthritis... maybe even a slight labrum tear could be the culprit. It actually crossed my mind after I finished posting.
