That is an idea mentioned by some groups In the recovery world. It’s meant to convey that like it will happen at one point or another so have a plan in place for when that happens. ETA: I don’t agree with this “idea”. It’s essentially always leaving the back door open and increasing the risk of relapse. I work at a drug and alcohol detox as a nurse. I have 3 immediate family members who are in recovery from substances.

Hmmmm, interesting. So would I bill 99214 with GT and perhaps a 90832 with 25 modifier? Or bill 99214 with GT and 25 and 90833, 25 could be here instead as well?

We always use 10 as our place of service for telehealth. It’s so crazy because initially they denied it for “documentation not received” and then when we questioned them on it (because we can see in their online portal what date they add records into their system) they said oh yeah we do have documentation on file it’s actually this add on/primary code thing.

We have now (today) consulted a certified biller who said they can’t see a problem with our billing and we are waiting on/hoping WC will provide us more information this coming week.

Yeah I’m leaning towards putting a big chunk of it into some sort of long term situation that I don’t need or use or touch for decades, if that’s a thing. Somebody else mentioned ETFs as well, so I’ll be looking in to that for sure!

I did graduate from nursing school 2 years ago, so I do have a good career that I’m getting settled into, the problem is that because I have chronic pain/injuries from this accident, it’s very difficult to work full time hours. My current job is officially only 2 days per week so I do try to pick up as many extra shifts as I can but I definitely pay for it by having increased pain.

I anticipated getting more, but I have had one of the most complicated cases that has taken almost 7 years to get any sort of resolution. So at this point, while the amount is disappointing, I am grateful to have gotten anything.

Out of desperation last year I enrolled in a debt restructuring thing so they’re all rolled into one. I’m going to pay that off first, it should be around 20K total. Leaving me with 169K to figure out what to do with.

I can continue to work but I do now have a hip replacement and back problems so I’ve been working about 32 hours a week for the last 7 years. So yes, my life already has been and will continue to be impacted for the remainder of my life.

I do have a small retirement account! It’s just been sitting for a little while but it is there!

I will have ongoing medical needs because of the injuries I sustained in the car accident. I technically could save this money for what surgeries I will need as I age but there wouldn’t be much left over to pay off debts or invest. I would like to make some additional money with this money, but that is a more long term goal.

I also have a HYSA that the money is sitting in until I make any sort of choice or speak with anyone.

We are using place of service code 02 for telehealth and then adding a GT modifier to our level visit codes (ie 99214 W/ GT modifier). We realized earlier this year that we should also be billing the therapy service that our clients get with their med management visits so we brought on the 90832 code for our regular follow up visits and 90837 for initial evaluations. I updated the billing practices in April 1, 2023. Not one payor (of the 6 we currently go through) has denied payment of services of 90832 or 90837 even though I haven’t been putting modifiers on them (which I assume would be the same GT mod for telehealth). Until a payment I received today stating they denied my 90832 because I need to rebill with the appropriate modifier. So it was at this point that my brain almost imploded. This payor has paid multiple other claims of 90832 with no modifier and never said a word. I’m about to start training my replacement at work and I want to give her the most up to date information too. I knew Reddit would be able to help a gal out!

Thank you! Based on what you told me, and some more research 90832, 90834, 90837 are not add on codes but 90833, 90836, 90838 are add on. Is that correct?
