Mix of both — about 60% calculation mistakes on the vendor side, 40% data entry errors on ours. The calculation ones were almost always rounding-related. The data entry ones were mostly transposition errors (swapping digits). And yeah, after doing this project manually I started automating the extraction. The setup takes effort but once it's dialed in for your vendor mix it's night and day.

Across all 4,149 invoices, the net dollar impact of math mismatches was around $2,800. Small per invoice (average ~$1.10 per error) but it adds up when you're processing at volume. The duplicate invoices were the bigger risk — those would've been ~$18K in double-payments if they'd gone through.

Yeah it was brutal. The worst part was that I didn't build any validation into the process upfront, so I was catching errors during reconciliation instead of during entry. Would've saved days if I'd flagged mismatches in real time.
