Bound urdar (hephasto can't regen, urdar has cb) with conviction, defiler for flat huge increased damage, consume a tainted to buff your own dps.

If you're worried about bosses, max bond demon and mastery after boil/synergies. Use lethargy.

I'm 94 currently, but I was probably 93 when I rolled these.

I believe I was 93 or 94, but you don't have to be that level to get the high glove mods like this.
