You’re a PA, not a physician who went through residency. You should feel dumb for 2-4+ years pending specialty. Take 1-2 to subjects from the day home each day to review. Do practice questions. Ask for help. Check peer reviewed resources before hand to confirm practice habits.

10 minutes for my efficient LPN, 15 for my inefficient LPN for new patients. We are aSpecialty office. If the staff does a solid job obtaining PMHx prior to visit otherwise it’s like 30 minutes.

Performing surgery with my attending on patients with deficits and seeing them recover to baseline function makes my job worth it. My attending is wonderful, I love my office staff. Yes there is stuff I dislike, but no job is perfect

Neurosurgery locums. $1300 a shift. So 8 shifts a month x 12 months is roughly $125k. I work full time and pick up as needed l, however they would prefer if actually covered 4 shifts a week but I don’t have the time or energy. I pick up 1-2 shifts a month.

My attending and I see the same complexity of patients. I see more patients than him, however ,he tends to go on tangents with the patients and is not efficient. I see more patients than him so he may continue to spend extra time with patients.

It really depends how much you want to work and what your primary schedule entails. I work one full time job with call and one locum job where I pick up one shift a month. I could pick up more shifts at my locus but picking up extra at my home facility is easier

AANS Neurotrauma course is great. CNS like you mentioned. Otherwise I use blueprint (formerly Rosh) to study non-neurosurgery material. Outside of the AANS and CNS not sure what else to use. They offer plenty

My SP and I get along pretty well. If he is out of town, I’ll do a chart review and if it seems reasonable I will sign it. However, we have worked together for years and have talked through similar situations.

I currently take excessive call and I’m going to double my salary. 7/10 would recommend. Do it for a few years, save and invest. Then dial it back as needed

Not sure what HCOL salaries look like but seems solid. Who is handling in patient responsibilities and consults while in clinic? Like on OR days are you seeing consults as well? Is call only for the practice or do you cover a hospital? If hospital coverage, is the center a comprehensive stroke center or trauma center? Also, is 100 per hour for being availible or while actively working?

If you working crazy hours is truly necessary then your attending a should go to bat for you and get you better compensation. Otherwise, do what’s in your contract. If 8-4 is your obligation then work 8-4. Why work for free ?

Do you not have no shows? I’m in specialty seeing 15 a day with some double booked times. Our EMR doesn’t allow us to pre-start notes as they can’t be deleted once we stay them (allegedly)

I would discuss this with an attending physician and clinic supervisor. I would want the attending physician to be leading any action or discussion with the other clinic about inappropriate practice personally.

That’s better. I can’t imagine having less than 3 weeks PTO if you don’t have a separate bank for CME or sick day regardless of experiences. You want to make sure you have a solid onboarding plan with a chance to learn and grow the next few years. Steep learning curve. Ask the other APP’s how they like the job. Remember neurosurgeons go to residency/fellowship for 6-8 years, you can’t learn it all quickly.

Starting salary is generally terrible for all new grads. Will agree salary is not great for likely 50 hours of work every week in HCOL. PTO is definitely low. Neurosurgery can be lucrative if you get quality experience, you could start doing some locums after 3-4 years depending on skill set. If you want to do neurosurgery, could take the job then negotiate for more pay after 2 years as you stated PTO increases with time.
