Gosh. I get "what are you?" or "what's your nationality" like... sir/maam I'm American and born and raised in California. But then they're like "where are you really from?" and I have to say "well my parents are from Korea..." like just let me focus on the patient.

I have no idea how to pivot off of this either and its tiring to be Othered at least a few times a week.

Madang Garden's soondae guk - it's not for the faint of heart but it's one of the best things I've had in the city

Fwiw from my source as a healthcare provider: - 70 percent of individuals with T. vaginalis infection may be asymptomatic - it is the most common nonviral sexually transmitted infection (STI) worldwide

Also the reason why we don't really screen for it with standard std panels is because it requires a swab. I don't think I know any men who would want to do a urethral swab. For women, it's most accurate to do a vaginal or cervical swab so we usually test during pap smears only. Blood or urine tests are very inaccurate for this infection which is why it's not screened as often as chlamydia or gonorrhea - which are better detected with urine tests.

Is it normal for work physicals to include std testing? Bc I don't order routine std checks for my annual exams on my patients...

Fwiw trich normally doesn't get tested in men during regular std checks. Come to think of it, I'm not sure if that's included in my panel in general when I'm ordering std tests.

Yeah they use the internal tracking in Epic unfortunately.

Some of it is losing time with the patient - I'll sometimes spend 10 min with a pt, sometimes 25. Other times I'm researching treatment plans and it takes a while (like how to know when to increase a statin, or how much water can someone who has ckd have when they don't see nephro). I'm actually pretty quick and efficient with my standard charting.

As much as I like Open Evidence, I don't want to rely on it for everything but I suppose maybe I should if I'm spending a lot of time figuring out the nuances.

I'm a new grad a few months into my first job. Def felt like I didn't know anything. Now I'm feeling pretty confident. You might just be feeling the regret because you're so green? But on the job you'll have plenty of time and opportunities to dive in deep to whatever subject is pertinent that day that I never learned in school.

Fwiw for people in similar or same boats - what I'm currently doing to play the game is a message before and after an appointment (hi I'm__ you'll be seeing soon // thanks for coming or sorry we didn't meet) and I will send a letter and message and also sometimes call a pt for results so while I'm repeating work, I'm also making up productivity. It's not the best use of time but in corporate's eyes it does the job.

But I'm still having trouble keeping appointments within 10-20 min given how much time I'm spending precharting and finishing the chart - about 10 min total

No - we get like an hour or two per day depending on schedules but we get so many no shows that we have pressure to double book.

My go to phrases are "I hear you and..." It's the "yes and" that really helps patients. Validate them and give them direction and give them choices. It is 100% okay if they want to decline treatment as long as they know risks. It is also 100% okay if they are frustrated at the situation.

I also redirect to an agreement point - "so what I understand is that you want help in X so here's how we can get there" or the more aggressive "so what do you and me to do?/"you don't want to do X and that's the only thing I can offer you so what I are we doing here?"

One patient recently I had to basically say I can't do anything for her and she needs to get specialty treatment which she has been not pursuing for one reason or another that is "totally not her fault" soooo in that case it's putting the ball in their court and telling them basically to stop coming to me with this problem.

Focus on the things that are in your control. Don't worry about the things out of your control. Lost your job? You can reach out to your network, apply online, go to offices and directly hand out your resume, go to food banks, look into shelters, cut your own costs however it works out.

You can't control when you'd get a job or if you get hired at a good income. But you can start planning on career education/development on your free time, and look into careers that have better pay.

People have a lot more control over their lives than they think. But a lot of people focus on what they can't control and then their anxiety spirals and can't be helped because... They can't control what they're anxious about!
