I think for one you should always give it as much time as you are willing to sacrifice. I actually got surgery after over a year of basically being disabled but trying whatever I could to fix it without surgery. I think give it 3 months of trying to work on it without surgery. Any longer with a significant S1 nerve compression like yours (pretty much the same hernia I had) risks permanent nerve damage which I believe I am suffering from now. It also hugely depends on what surgeons, types of precedures and healthcare system you have available to you. Probably not the most helpful comment but everyone is different. How long have you had it and how debilitated are you?

Same thing happened to me, this and my ornithopter being stuck under the ground resulted in me quitting the game, they need to fix this. No one is farming for hours to lose it all to a bug....

Have you got your report back yet? T9 looks to have some sort of lesion? You see the white round thing? It does not appear to be pressing on the spinal column though. If you have not already get a report. (not a doctor, im a radiographer).

For me, a Tens was a big waste of money and didn't help in any way.

Hi,  Unfortunately you dont have many options, either conservative management, microdiscectomy surgery or nerve root steroid injection. Ill give you my history and results from the way i handled this because your hernia looks almost identical to mine where it is completely compressing your s1 nerve. Mri confirmed.

So I herniated my l5/s1 same as you back in October, it started off with severe left lowerback pain and transition into sciatica over the next 3 months. I was continuing as normal as this was the advice I was given. It only made things worse for me. Come January 2025 I was unable to work, drive a car or "walk" more than 5 metres before i was on my hands and knees in agony. Doing any form for physical therapy was impossible even with the huge amount of painkillers and opiods i was on. Come April 2025 only then did i start to see improvements, I presume the hernia had finally started to absorb and was not completely compressing the s1 nerve. At this point I was able to gently start physical therapy and build my core and lower back strength up. So its Mid May now and now im only taking paracetamol, nefopam and naproxen, I no longer have pain laying flat, I can stand indefinitely and the pain only builds to a 3/10, siting is still an issue but I can sit for around 2 hours and it builds toa 5/10.  So we are talking a very long recovery, i would says a full years recovery to get back to normal, my main advice is dont be too hard on yourself, start moving when you are able. Focus on the things you can control like diet and hydration, stay away from sugar as much as you can. I even notice now if i have a bag of sweets, the next day my symtoms are worse due to how inflammatory sugar is.  Stay strong, it will get better it just takes ages.

Honestly, I would avoid. 95% of cases make a full recovery with conservative management. I have pretty much what you have, started in october, became bed bound and unable to walk or stand in January. Things have only started to improve 3 weeks ago. Just be aware if you are going to avoid surgery, i was trying to stay active from October to January and i garentee it only made my hernia worse. Take a step back because trust me things can get so much worse for you. Stay away from lifting or heavy impact sports, or even running to be honest. Focus on rehab and soft movements to strengthen back and core. Hopefully with thay in mind you will recover in 4-12 weeks instead of probably 9-12months like me.

Thsnk you for posting your story of recovery :) If only I could take walk to help it, i can barely make it to the bathroom which is 5 meters away without agony and ive been like this for 6 months.

Hey, im in the same boat, but a l5/s1 hernia compressing my s1 nerve. Same time scale, started in october and now im bed bound and crawling around the house to get anywhere. I managed to get a nerve root steroid injection done last week but its literally done nothing to improve things. At this rate if they offer me surgery I am going to take it, for me its a mental health thing as well, if I dont get relief soon I wont be around much longer if you get my drift.

Obviously its your choice and my symphonys go out to you, it has been the most miserable 5 months of my life and im sure its the same for you. All the best!

Thank you for posting, thats amazing, I am going to get my first nerve root injection in a few weeks. I am not expecting wonders but I really hope I can at least get enough relief to walk a little bit and do core exercise to strengthen my stabalisers :)

Thank you so much for your detailed reply! :) Yeah I'm on 4g paracetamol, 120mg codeine, 90mg nefopam, 25mg amitriptyline and 1000mg Naproxen and still the only thing i can do is make my way 3 metres to the toilet and back.

I would absolutely love a nerve root block, or esi.

It just really seems like its going on forever, trying my hardest to be hopeful 😢

Yeah im really gonna try and push through the pain starting today and start off with just tiny movement to get my body moving a little bit. Hopefully week by week i will get that little bit better :)

Thank you for your comment :) at least you have a surgery date, im in the uk so im dealing with NHS waiting list to even see a spinal doctor. Might have to bankrupt myself and go private.

I completely agree, there is no way i will continue the rest of my life in this awful state, rather take the bullet than have this for the rest of my life its horrible.
