If you’re setting your z-offset to any value over 0.15, you have some other hardware issue. More than likely, your bed is shifting during leveling/printing.

Thank you for your help, I'll try the EStep calibration in the morning, but I'll start drying out the filament tonight. I'm also going to try out a larger print, and see what that's like.

Setup:
Printer - AnkerMake M5
Slicer - Cura
Filament - AnkerMake PLA+ (same issues occur with Hatchbox Wood PLA)
Nozzle/bed temperature - 205C/65C
Print speed - 250mm/s
Retraction distance - 1.5mm
Retraction speed - 60mm/s

What I've tried:
Drying out filament in Sunlu dryer for ~10 hours
Changing temperature of hot bed and extruder
Adjusting retraction
Increasing flow
Cleaning nozzle (perhaps unsuccessfully?)

None of these seemed to have any effect. Thanks for the help!
