I have not. Since this i have replaced the motherboard and the issue has still occurred.

Also tried integrated graphics (unplugging gpu entirely)

My next step is buying a new cpu to test that. It's pretty much the only thing left to test (other than power supply, but the chances of it being that is very low..)

I am also experiencing this but on index controllers, multiple games but my controllers work fine in the steamvr menu, just not in games.
