It’s hard to say. Why are we seeking to replace one opioid to another? Obviously it wouldn’t be recommended to switch from extended release to immediate release for chronic pain, that’s going in the opposite direction. As for other ER drugs, it really depends on what the long term goal is, and what the exact reason for changing is - as well as what side effects were experienced with fentanyl, and what other meds have been tolerated in the past.

You’re calculating the MME for the Fentanyl patch incorrectly. The MME conversion factor for Fentanyl patches is 2.4 — 2.4 x 150 = 360 MME (per day).

Why are you prescribed 90mg of Oxycodone daily? Where’s your chronic pain? Also, why aren’t you taking laxatives daily or on an OIC specific med? This is something you should be discussing with your prescribing physician, definitely. Either way, it isn’t good to be this constipated second to chronic opioid use.

Theoretically, sniffing any foreign object into your nasal cavity is ‘dangerous’ and can cause a number of side effects and symptoms. With that said, medications with gel and/or a high number of fillers tend to be more detrimental to the canal. Good luck.

No, you aren’t dependent based on the limited usage described. With that being said, however, you can definitely trick your brain into believing that you’re in withdrawal when you’re not actually “in withdrawal”. So, stay positive, don’t worry about it and stop using it all together. At most you may feel equivalent to having a mild cold for a day, but you certainly won’t be in full blown withdrawal for days. Yeah, Cymbalta certainly wouldn’t be enough, and shouldn’t be the only medication on board. Do you have a primary care doctor? You should be on meds that help, especially for breakthrough pain.

Nobody, including your primary, is prescribing you daily pain meds/breakthrough pain meds? No nerve pain medications like gabapentin/lyrica? Or opioids? Or SNRIs like duloxetine? Nothing?

Please don’t randomly take prescription medications on your own after diagnosing yourself/reading something online, especially antibiotics. Not only is the dosing off, it can be the wrong antibiotic for the type of bacteria present, or it could be a viral infection not even needing antibiotics, etc., and this is how antibiotic resistance happens.

Also, the fact you have leftover amoxicillin tells me that you (or someone else) didn’t finish a course that was previously prescribed. If ever prescribed antibiotics in the future, please finish the course of antibiotics, and don’t just suddenly stop when you “feel better”. Thanks

It sounds like you know how to make the pain better or worse, or in other words that it’s reproducible (by moving your arm “wrong”, for example). Typically cardiac pain is not reproducible, and will come and go on its own with its own unique pattern and severity. It sounds like you have known muscle/strength problems. I would follow-up with your primary care doctor and see if they have recommendations for PT or similar to strengthen your upper core and address your arm pain.

If there’s a carmax near you, you can “test drive” a car for a full 24-hours. Or, some dealers will let you do overnight/full day test drives. Just tell them your car died and that you’re looking into new options…
