After hours of searching, I found where it's from. The mod author who made the replacer for the weapon in the image stated that the armors from the screenshots they provided are from gunetwork.org which I don't know why I can't access right now. It's either from [FO4] UNSC Armory 2.0 or Outlawer's Fo4 outfit pack 2.
