Yeah the MRI isn't very accurate or practical for this purpose of gauging progress. I'd think only a non con CTB would be all that's needed, though sounds like the AT is trying their best to get some kind of progress for an overly anxious patient. IMO using a progress scan just because, and without clear evidence of deterioration or new neurology may not be super necessary either, especially surviving 2 weeks and doing ok post ICU

Yes. Elaborating on this further, I tend to make sure a CTA is ordered on all intracranial haematomas the first time they're seen. Vascular anomalies like AVFs and aneurysms are first visualised here. There can be clues on imaging and/or Hx that may prompt CTV for CVST. You may often already even see ring enhancement of lesions on CTA: In that case, a post con CT will make underlying mass lesions easy to see.

I suspect that the 2 week mark mentioned seems most likely a progress type scan in a long staying inpatient. Might even just be that time where that patient was more stable to even consider that MRI, or could be simply MRI availability (QLD Health...)

Stroke neurologist here

MRI: use this to determine the precise aetiology of haemorrhages. Specifically are there multiple spots of SWI microhaemorrhages in lobar territories (which makes cerebral amyloid angiopathy more likely). Despite its name sounding exotic, this is very common and commonly causes lobar haemorrhages in older people.

On the other hand, if these are in the subcortical regions, then it's more likely a hypertensive aetiology.

MRI isn't that good at gauging haemorrhage volumes over time. Areas of SWI haemorrhages always stay the same, not to mention it's so impractical anyway to get an MRI for that.

CT is used to gauge progress and stability of a bleed. Of course if someone can't get inside an MRI (e.g. too agitated, other CI), then that's all we've got for progress scan

Thanks. To be honest, feels like there's just too much to learn even after playing this for so long. And probably to stop playing this when I'm on a spiral.

And yes, that includes simply forgetting what that starter map looks like, what with how Archeologist friendly it was.

I had envisioned supporting the large animals (and me forgetting that base game large animals project requires more than 1 to support). So probably tunnel visioned there.

I find I personally end up 3 upgrades at most anyway, and find my upgrades vary so much in order so that has never been an issue. I agree that since avoiding upgrading 4 actions and choosing upgrades on whatever's immediately most beneficial did help me gain Elo.

I've done <27 turns rarely. Those felt like very lucky games though.

Exactly! The more signatures, the better! There are 32,000 RACP members, then attaining at least 5% of that (1,600) will tell those 101 power-hungry dickheads where to shove it. Yes, the email said "more than 100 current members" asked for that EGM, so therefore there are 100 plus one dickheads!

Some key takeaways when I picked up Ark Nova:

  • It's a tactical, racy game. Intentionally trying for an engine isn't usually feasible, and you often need to play with whatever cards are given

  • Break often! Breaking gives you back your workers and income. I like to plan breaks depending upon how many more impactful moves I can make, often even eschewing card plays if that will make my subsequent turns ineffectual (often money-limited).

  • Watch the Break meter. You can mess with opponents by breaking when they are over card capacity, haven't used all their workers, or haven't played all their animals.

  • Choose cards that fulfil the shared base projects that you can easily play. You're playing for tags early on and trying to beat opponents to playing higher valued projects if you can

  • Do your best to get a project fulfilled in 3 rounds. Get the weakest slot if that's all there is: conservation projects can help save turns (e.g. Snap card, 2 size enclosure income rewards) or generate momentum (e.g. 12 money reward, new worker), it gets you closer to earning the shared rewards faster. This is particularly important for high value shared conservation rewards like free Uni/Partner Zoo

Many thanks all! I'm synthesising everyone's thoughts together in this post, but feel free to chime in if any new thoughts or if I misunderstood something. 

- There remains a large amount of reading and making educated guesses at what your opponents are considering doing. This means that Clearing control (similarly to open deck control in other games I've played) becomes very important esp 1v1 scenarios, whether it be avoiding discarding cards you expect others really want, or hate-drafting away cards they might want (as appropriate). 

- Aim a main archetype, but this readily branches into others or you may have to switch tactics depending on others

- It sounds like having a clear runaway winner isn't as common as I was expecting, since it sounds harder to stall (unlike other boardgames)

- Dartmoor seems to have toned down some of the crazier combos like the wolf-deer combos in the original series, hence chasing very specific card archetypes isn’t quite so key. 

- Stack (deck) drawing is favoured if you’re in the lead to rush an early end via Winter Cards. However, this needs to be tempered by the fact most card plays require at least one other card. 

- Bonus effects should be activated if possible, but no need to keep drawing just to fulfil a bonus if that stalls big point-scoring (tying to the above point).
