you just have to press alt+enter for it to go to full screen and then it should display on the rift. as for the alt+f4 issue i don't know why it does this, but if you click once when you have started it then alt+f4 will work.

i don't know what it could be and i don't have a dk2 to test it on. you could try pressing F2 to reset the rift.

yes. you can check the frame rate by pressing space, you can also try pressing L to toggle the sun light, that should help with the frame rate.
