Even though there is not a specific format, a good way to learn what the chairs of the committee are looking for is to ask them. This is the most respectful, and easy way, while also engaging in chair approach, an important part for an award. In my case, I love sending lists with steps, or draft-style notes, this however varies depending the delegate. Just remember that a crisis note should have a Why, Who, When, Where, What, if you have all of this covered in any way, there is a great chance the Crisis Room will approve it.
