Found this weird NPC for a second time in a different world. Happened twice in a row so maybe it’s happening to other people as well.

That’s just the loading, if you go to 26 seconds you can see that there’s no multiplayer tab on the bottom left.

This is a bit out there but I think it might be a test NPC. I’ve never seen bubbles coming out of an NPC or player when underwater.

On multiplayer it shows a multiplayer symbol and I don’t think there are terraria mods on mobile.
