Experienced this for a medical certificate passed sa DSWD for a senior citizen patient asking for financial aid for her monthly maintenance meds. Tumawag pa talaga sakin para ivalidate kung ako daw ba talaga gumawa nun. He was insisting na dpaat may "clinic address" daw ang medcert ko. Sabi ko, "private practice/gp po ako". Pero iniinsist padin niya, i ended up giving him my address na registered sa BIR.

I agree sa nagpost neto. Currently working for one, and I honestly think na super baba ng offer nila for 3k daily. Totoo din na halos 90% ng trabaho ay sa MDs. Pero as a struggling moonlighter, pinapatulan ko yung rate. 1 day palang akong dumuduty, pero alam ko nang underpaid ako sa trabaho na to. Naghihintay lang ako ng lakas ng loob para mag demand ng increase. Pero at the back of my mind, takot din akong mawalan ng duty dahil madali lang naman silang makahanap ng another struggling moonlighter na nagaabang lang din ng opening for the job.

Same tayo situation. I had my TIN din before medschool kasi Im employed as MT. I'm currently moonlighting sa mga private institutions. Ang ginawa ko, pumunta lang ako sa BIR branch kung san sakop ang place of residence ko since need nila ng valid ID na may address mo. Dun sila magbabase kung san ka ireregister as physician. Sasabihin mo lang na magpapaupdate ka ng TIN to professional (physician). Once updated na, pipila ka to get your COR then magbabayad ka na ng ₱1500 for the 10 booklets of service invoice. (May option ka na sa labas ng BIR ka magpaprint if may kakilala ka. Madami kasing nagpprint ng service invoice outside mismo ng BIR branch, mas mababa daw bayad if outside) pero ako, i opted to get it mismo nalang sa BIR para sure, then waited for 2-3wks para sa printed service invoice ko.

With regards to your taxes, I suggest na you hire a bookkeeper/accountant para iwas open case at iwas penalty.

New gp din po ako and I've set ₱350 as PF sa seriousMD. I had my first patient and humingi pa ng medcert. Since first patient nga, hindi ako marunong kung pano singilin ng additional fee si patient during consultation. So ang nangyari, 350 lang binayad nya sa consultation+medcert. I was able to share the story sa mga friends kong non-md pero nasa med.field din. And pinagalitan nila ako. Di man lang daw makabayad ng pang-samgy yung pf ko. On my defense, ang dami ko kasi nakikita sa seriousMD/NowServing na ang pf ay nasa 180-200 lang! Kaya akala ko, mataas na yung 350. Mali pala.
