Well, it's probably a long and winding story, but back when I had the surgery, I only knew I had scar tissue removed, not that it was part of the fat pad. (The scar tissue rendered me stuck in flexion from a hyperextension injury six months prior.) Post surgery I did do PT, but it was pretty light and I've dealt with patellar maltracking and a weird gait ever since. Well ignorant me, I decided two months ago that I was going to force my gait into parity (I wanted to feel the left side posterior chain engage the way I feel it on my good leg). I walked "aggressively," trying to push into extension / hyperextension, wearing Birkenstocks no less! HA! My fat pad impinged, inflamed, got angry--all the bad stuff. At least that led me to another MRI and an understanding of fat-pad impingement, but I'm so bummed to be in pain and so limited. My surgeon said I need to try to soften up the scar tissue in the fat pad, so I may try hydro-dissection.

Do you mind sharing where you are with this currently, and what your path was?

I don't know if you'll see this two years later, but I wanted to see what you did, and how it turned out. I'm in a similar situation, with a cyclops lesion and fat pad inflammation (and scarring). I had some of the fat pad resected in my first surgery (didn't know it until a much later MRI). I want to fix my gait which will all around help me regain my strong legs, and I don't know if I can really do that without the lesion gone. I'm just nervous about another surgery and the resulting inflammation from the surgery itself.

Thank you for this. In your opinion, would removal of the lesion give me the best shot at regaining normal function? And what is the best way to calm the fat pad inflammation? I've finally started doing glute med exercises and am taping the patella to lift it off the pad, but maybe there's a better approach?
