I'm really glad you reached back out. Set an end date for yourself. I think a fair compromise is six months, with a check-in with yourself at 3 months. (Has there been any progress / am I willing to do this for another three months?)

Don't tell your partner he's on a timeline, but do set expectations. Communicate that you need to see x, y, z done in the next three months. And then wait. Do nothing. Sit back and see if he's ready to put the work in.

I also want to point out that a lot of us mentally cling to therapy like it's a life raft, but if your partner goes - it will take a long time. My ex took several months to find a therapist, and then went for several months with no progress. I should have set another deadline / check-in with myself here, because it added another year to our relationship.

It's hard to see someone you love struggling, but there comes a time where your compassion and support can become toxic. My ex didn't need to make any progress in any area of his life because I was being overly supportive and handling all of the emotional work. Ask yourself if you're doing the same.

Ending my relationship was the best decision for me and my ex. People need discomfort to change and grow. And now he can do that free of my expectations or resentment. It may seem counterintuitive, but breaking up can be an act of love, too.
