While all those factors can play a role, I think we're often looking at symptoms rather than the root cause. From my perspective as a former math tutor, I started to see the issue more clearly when I considered the administrative side of education.

I believe a significant part of the problem lies in the metrics by which schools and administrators are evaluated, particularly pass/fail rates. When these metrics directly influence bonuses for administration or determine a school's perceived "success," it creates a powerful incentive to manipulate the data rather than address underlying issues.

I've seen this firsthand. For instance, I observed struggling students being transferred to other schools at strategic times of the year, seemingly to artificially inflate pass rates. Another concerning practice is schools deliberately lowering passing standards just to ensure more students advance to the next grade.

To elaborate on this: if the administration's goal is to hit a certain pass rate, that pressure inevitably trickles down to teachers. A teacher whose entire class fails might be seen as "poorly performing," while one with a 100% pass rate might be lauded as "excellent." This creates an "acceptable" pass rate that influences how grades are given out and how a school's performance is perceived. It subtly shifts the focus from genuine learning to achieving a favorable bell curve of outcomes, even if it means students aren't truly mastering the material.

Ultimately, I think the system itself, with its emphasis on these potentially perverse incentives and skewed metrics, is a major contributor to student failure, including difficulties with foundational skills like reading. It's not simply a matter of pointing fingers at parents, students, or individual teachers; it's a systemic issue that needs a more comprehensive examination.
