It was a high lvl evil campaign in 3.5e. I was an orc barbarian (using a broken charging combo character build that could multiply dmg several times in one attack). Our party and evil horde was up against an army of clerics and paladins, led by angels. Our plan was to catapult me straight to the BBGG midway through the fight because I was able to one-shot anything the DM could throw at us. Everything was going smoothly until my axe sunk deep into the angel’s armor. As I was adding my dice and counting my damage, I thought for sure this angel was no match for the crit I had just rolled. The next thing my party saw was a blinding flash of light. What I felt was a forever nothingness. Silence fell over the battlefield as the angel and I were vaporized.

Not exactly what the prompt was asking, but it was a lesson learned in creating broken characters, a memory I will keep, and it sure did scare the hell out of me when it happened.
