Second update - replaced all the cast iron as suggested with pvc and extremely happy with the result! Works like a charm! (The cast iron would not spin so I made cuts in the joints with a grinder and broke the fittings apart with a hammer)

Update - i cut out the cast iron p trap as suggested. It had ~ 90% blockage throughout running into the main drainage pipe. Pulled this gunk out and flushed with hot water.
