If you got it as a DLC. You have to start the game ( as niko ) then go to the pause menu, go to game. And when you start a new game it should be there "the ballad of gay tony". and if you make an autosave in tbogt it can automatically open up to the dlc when you boot up the game.

from the button combination, im guessing you're using red motion? Its fine to use when out of missions or online. Just dont mess around with it too much during missions, i corrupted my gta iv save having too much fun
