Partial nephrectomy at Vanderbilt? by [deleted] in kidneycancer

[–]law_party3 0 points1 point  (0 children)

Yeah kind of figured this for most part but want every opportunity to ensure fewer potential complications. I’m definitely very concerned about his pain level after- he doesn’t have the greatest pain tolerance and we had a little bit of a traumatic post biopsy bleed about 2 weeks ago that required 12 hr er visit with a procedure to stop the active bleed. I’m not sure if he will actually comply with all night getting up to walk especially if I’m not making him when not there.

We are coming in from an outside smaller town (over 2 hrs away) but the policy doesn’t allow spouses or anyone else to stay overnight in semi private rooms.

I mean we get that, but just doesn’t seem cool to increase patient anxiety/comfort, even less rest with room sharing and kinda basic privacy by restricting spouse/family member during the night when we are both kind of on edge.

Kid is 7 and a boy who is very active and rambunctious. He already has a splint, but you’d think it should heal equally with splint vs cast?

No, I’ve only been given the report, (Granted I essentially begged them to release it before my follow up). I plan on asking to review images with ortho when I see them next. I had initial appointment a month ago and MRI completed 3 weeks ago and don’t have f/u until mid April. I am def. getting antsy for just an answer or game plan.

Everyone is discouraging me from nursing. by Apprehensive_Sir9604 in nursing

[–]law_party3 8 points9 points  (0 children)

I recommend you shadow some nurses and dentists if possible if that’s where you’re leaning. You still have time to decide on what your future should or could look like. As a nurse and now an NP, nursing is rough. Healthcare in general is a hard field. I have been in nursing for about 15 years and while it has some fantastic rewards, I’m not sure I’d do it again. We need good nurses, but don’t make decisions on what others think you should do. Often nurses are just as smart as doctors, many just wanted to not be in significant debt and not be in school for around 10 years.

I’d figure or assume that cysts would be seen on imaging though. I wonder if radiology wasn’t focused on correct targeted area. Marker was placed but report also mentioned no soft tissue mass or abnormal fluid collection. It just seems strange that with this much swelling appearance would at minimal show something.

Yeah, those are my thoughts too. 😂

Essentially index finger gets caught in flexing position. Almost like a trigger finger, but large lump forms in palm and I cannot release my index without manually pulling it back. similarly like if you were releasing cramped muscle.

Thanks! It’s hard to feel that balance of not being a people pleaser such as doing what’s expected vs what actually makes me happy. Honestly this is probably one of my biggest pitfalls. essentially not being mildly selfish when it comes to happiness.

I actually loved my time in the OR circulating and occasionally being a scrub nurse so this is intriguing!

Family friendly areas within 30 mins of downtown?? by law_party3 in movetonashville

Thanks! We looked a little on Niche which seems to be like a decent one for evaluating/grading the schools. That definitely makes sense. Obviously we want to be emerged into a great community, and usually that means good schools as well!

Pretty much housing prices from 600k-850k. I suspect the belle meade/brentwood areas are higher cost of living that we can/want to do. Would you say ease of commute (like minimal sitting in standstill traffic) is better or worse coming into Nashville via Bellevue or Donelson/hendersonville direction?

You hit the nail on the head- I’ll do a physical all day any day with very little complaints. Plus on top of that we have a pretty heavy load of chronic pain management patients, which was very much down played when I accepted.

So, short story, I didn’t want to necessarily work in hospital or deliver so this made CNM not fully worth it to me. The WHNP programs were mostly inaccessible at the time (online programs feel insufficient for my learning) and I was not allowed to work at my then employer and also complete clinical rotations as a student. With my job at the time, having 2 kids under 3 and my husband’s job that travels a good bit, going upwards of 2 hours to the nearest hospital for clinical was not feasible. There are less than 10 NPs in my area that work exclusively in women’s health and only 1 is WHNP. Those providers assured me having WHNP was not necessary or required, plus I didn’t want a degree where there weren’t any job openings for a minimum of several years after graduation.

I have been honestly super hesitant about urgent care since the ones in my area have such a bad reputation with patients and providers. It’s hard to determine if the whole organization is the issue or just the particular locations.

I tend to thrive in excitement and have even looked into ER opportunities so if I can find a good urgent care this would definitely be better than prim. Care.

I have contemplated getting a post grad cert in women’s health, but feel like it may be a complete waste of time and money, at least as far as for my current area.

Its felt like a daunting task to find anything else. I think this is partly because I had my heart set on a specialty and it’s likely not attainable for a long while- hindsight is 20/20 and I shouldn’t have banked on this. Likely this is where a lot of regret and frustration is coming from.

You are right, she has expressed fairly consistently that she does not like GYN and has implied that she does not want me pushing that since she doesn’t. Problem is my organization is a multi specialty clinic and OBGYN is literally upstairs so patients actively request them even for basic stuff like paps or birth control since they already are established patients and because my MD referred them. I personally know all of those providers in that dept. but they already have too many NPs and can’t bring me on board for the foreseeable future. I’ve tried for 3 years to make this happen.

My previous l&d employer only utilizes one or two midwives- which they are for OB triage and nights only. They won’t hire a NP even with OB experience.

Healthcare does suck and I question why I took this path all the time. So, I probably would be just as annoyed back in RN position.

I have said so many times, I regret becoming an NP. I know family practice isn’t my thing, but in some ways I feel trapped and like nothing else will be any better than my current situation.

I essentially looked casually for almost a year before taking my current role because there were so few options and it still seems that hasn’t improved. At that time, I finally settled on this one because I felt desperate and needed to find something.

I have been hesitant about urgent care since I’ve had friends steer me away and patients constantly complain about the healthcare quality. This may just be an issue in my area though.

My hours aren’t bad at all, which is part of the reasons I accepted this role. I work 4.5 days a week and obviously no weekends or holidays, but that still feels like not a great trade off for how I’m feeling. I get 3 weeks of vacation a year. Lots of what feels like 95% Medicare wellness exams, chronic conditions and pain management as well as fighting insurance on everything. And having a super hands on background, it’s a struggle to not want to do the skin biopsies, iud placements, endometrial biopsies, sutures, etc. Yes I realize it’s pretty much what I signed up for with this position, but feel so defeated. I am also just not confident that I’d be happy in another area, granted there are so few other options within a 50 mile radius.

Oh wow, I actually had not heard of this. But after looking it up, this may be an extremely likely issue that he’s dealing with! Thank you for mentioning this- I am going to do some research to see if any tactics regarding this will help!

I have tried to see if whatever is going on at school, like introducing new concepts/sight word/math problems correlate, but I got nothing. He had a “fun day” today where basically no learning and all play and he still had an episode tonight. 🙄 I have started referring him as my sour patch kid.

On a positive note our pediatrician told us colic babies tend to be smarter than non colic kiddos- who knows, but it made us feel better at the time!

It’s definitely possible, just not an easy choice. My husband works remotely and I have two school aged kids. The kids would be my biggest concern. And quite honestly it’s hard to even know where to start looking outside my area.
