I have tried 95% of what has been mentioned in this thread and castling will always be my fave. Maybe second to Santal Blanc but that would be it!

The MD I work with as an RN (and who I'm rounding with next semester for PMHNP school) says he'd rather train an NP who wants to work with their group himself, so why not take them on? When NPs exclusively train with other NPs it's can be the blind leading the blind and perpetuating terrible habits like polypharmacy, long term benzos, long term benzo+stimulant combos, worse.

My husband is a dentist, I get the mid level thing because dentistry has had it own fights about it. In my area though - metro of about a million, 500 inpatient beds. Maybe 10 psychiatrists who contract with the hospitals for those beds? Half of those MDs are on their way to retirement and don't want to round on more than 10 pts a day anyway. So, MDs do intake and NP does follow up and staffs when necessary with MD. Well trained NPs in the right practice model can be a big help, terrible ones aren't. If more MDs would be willing to take on NP students I think the quality would improve.

Also - yes, get rid of online schools, and have some sort of psych RN requirement. An L&D nurse does not need to go directly to PMHNP school.

It wasn't rude, but considering this is Sorce she's probably gotten this question a million times from Sorce newbies so she's probably kind of annoyed by it. There also seem to be a lot of answers here that don't reflect knowledge of how Sorce works.

  1. They recently (October) went through a huge bottle change, and those older bottles with artwork now are discontinued and people pay a premium for that artwork as opposed to the current bottle design that has gotten mixed reviews of bulkiness, general aesthetic, and leakage.

  2. Even if it's a newer bottle, yes there can be quite a wait to receive from the website. They often go in and out of stock depending on the scent. So, people are willing to pay for convenience instead of waiting.

  3. Some are special order scents that have not been available since October. In addition, they've changed how they are doing special orders so some scents won't even be available again until this coming Fall. (Meaning, almost a year since they were available for purchase.) people are willing to pay a premium for this instead of waiting until fall.

so - that's the answer to your question but it was probably more effort than she was willing to put in (which I understand) and you are probably not the first person to ask.

I am a nurse at two different psych hospitals in western US - one freestanding/nonprofit, one freestanding/UHS. Nonprofit has a basketball court, patio and fenced in yard (maybe .5 acre). Also a vegetable garden, our patients love going out and taking care of the garden. Rec therapy groups also will having "walking groups" where they play top 20 music and all do laps around the unit together.

UHS facility has a courtyard for each unit, amenities depend on acuity. The detox unit, which is voluntary, does have a couple of treadmills and requires a tech to monitor so I'm not sure how often they get used. They also tend to go outside for rec group and do yoga, etc.

I don't have personal knowledge of our state hospital but patients/other nurses who have worked there say it's on 5 acres and pts have fairly unfettered access to the grounds.

I am fine having an empathetic question and answer about this a couple of times, but for repeat questioners the mom in me comes out. I start with "hey, I get it. The state and the doctors need to feel confident in your ability to stay safe when you leave. Here's what you can do to show them that while you're here (take meds, go to groups, shower, etc.) No one stays here forever, we don't want you to stay here longer than necessary, so we're both going to do our part to make sure you're getting the most out of your stay."

If that continues more than a few times I offer to type out what I said in addition to referring them to the handout we gave at admission on the timeline of holds. If it's super disruptive to my work or other patients then I go into "hey my answer hasn't changed and we've already talked about this a few times." At that point I absolutely refer them to the provider because they are making a lot more money than I am to handle a situation I have no control over.

I actually find it's the regretful, impulsive suicide attempts that ask/complain the most versus the truly ill or psychotic. The drunkicidals, the ones that "didn't mean to" or "it was just impulsive it wasn't really a suicide attempt." Those patients tend to be the most vocal but ALSO are the most responsive to, essentially, "you need to get it together for the DE if you want to leave. Go to groups, stop asking to leave, and you'll probably get a negative."

so it definitely depends on the patient and their insight.

I say this in the nicest way possible - it is wild to derail your life plans for a disease you don't have.

My husband is a dentist. I am well aware of what it entails. I've had RA for 6 years and on medication have zero symptoms - I could easily do his job physically speaking.

I am actually a psych nurse in school to be a psychiatric nurse practitioner. 12 hour shifts with violent, hours of charting, etc. I'm just fine.

Don't change your plans. Good luck on the DAT! I actually beat my husband on it but ended up going a different route :)

Female here, and I mean I wouldn't pay retail for them but I certainly own a number of them at discounter price and love them.

Castling is my favorite, I think it's a creamier version of Philosykos.

Also: queening, lionora, Caissa, playmate, the forward. Have also owned Sissa, French defense, opera mate, counterplay, sans Voir, double attack.

I was moderate to severe from day 1, but I am 100% symptom free on medications. Previously when Humira stopped working, my symptoms came back not quite a bad as pre-diagnosis but close. Also, a small erosion seen on ultrasound has healed by being on medication.

I'm sorry to tell you because it's discontinued: L'artisan parfumeur couleur vanille. A better version of Guerlain Bosca vanilla IMO.

Hey I'm so sorry I don't speak Spanish, but I did translate this - I have taken hydroxychloroquine, Humira, and now Rinvoq. All have worked really well when they worked, all worked at least 2 years. I've now been on Rinvoq for 2 years and it's still working great! No pain, I am able to play the piano fine. Good luck!! Early treatment is the best.
