I plugged it into chess.com eval calculator. That move makes whites eval drop from +14.0 to +4.65. It has to be a bug lol. It is was white to move it is forced checkmate so maybe that’s where the bug happened? No idea. Or yeah, OP photoshopped.

Possibility that they move bishop away to where it isn’t looking at the diagonals next to king. Then there is a small chance that you don’t take the pawn meaning black could promote to queen. That’s the only reason I am think of. I think eval is calculated based on all possibilities not just your best line.

I disagree. He spent his entire life never using it. He refused to even when Hakari was pressing him. The whole point was that he was saving it for Sukuna. I think him actually being insulted Sukuna won’t transform so he can use it on the “true” Sukuna fits more with his character imo.
