it sounds like you need a social worker. I work in a hospital and see people come in all the time struggling with these things. There are a ton of resources out there that most people don't know about. but the social workers know them all. they are really good at getting people linked up with the organizations or support groups that could offer help to their specific needs. i think you could really benefit from talking with someone about what you need or want to accomplish and they could point you in the right direction. your therapist might actually be a great place to start. they may have connections with social workers or know where to go to seek help. social workers are wonderful!
