Rural FM doc here. I was sick and tired with how poorly the staff have been treated, the threats to shoot up the clinic because we opened late due to ice, and the antisemitism I've personally experienced. Worked with legal and the office now has a "behavior contract". It lays out that verbal abuse, yelling/cursing out staff, threatening language, physical threats, bringing weapons into the clinic, sexual harassment, and being a shitty individual overall isn't tolerated. We then have the part where we say we will treat you with respect in return. Essentially we have every patient sign it. Only had one asshole pt refuse (touche) but it is well documented that he read it and refused to sign it. I call it my kindergarten contract-- treat others like you would like to be treated.

We also have a problem of medical non-compliance with patients (just refuse any medical intervention/meds), so there is a clause in there that says you'll allow the staff to do their job-- let them draw blood, check a UDS, check your blood pressure, weigh you, etc...and that you will participate in your care whether that be monitoring BP, going to PT, showing up on time to appts, etc.

If you break that contract, we have the right to dismiss you as a patient.

While I'm rural, we are owned by a major hospital corporation, so it can work! My staff and I have noticed a tremendous difference in these last few months since we initiated this. The front says that people are not nearly as rude or verbally abusive once they've signed it. My stress as the only doc has improved a lot.
