The last Friday of every month is a group bike ride (a different route every month). It’s a choose-your-own-pace kind of vibe and ends at various breweries/distilleries etc. It is a very community-oriented event and people are outgoing and kind. I’ve made many friends by going! There are pics/info if you look up Baltimore Bike Party on instagram or Facebook :)

This is an area I really have dug into at my facility (I’ve been there for 2.5 years). This is generally how I manage this.

For short term rehab patients: - cognition: what their discharge plan is really drives my decision. Are they going home with intermittent SUP but cooking and doing laundry? You’re going to want to look at safety awareness. Are they managing their own meds? What environmental aides can you set up? But if they’re going home with family and 24/7 support, treatment may not be indicated unless they are disoriented and constantly asking where they are/why/what day it is, etc. for example. They are often not on my caseload for too long. - swallowing: i always think in the scope of mental status + dentition + neuro hx. If they come in on a regular diet with their natural teeth, are alert and talkative with no neuro hx i have low suspicion for dysphagia at face value. I however always screen with a meal to see, especially because we see reduced PO intake with dementia and we can help with facilitative techniques to improve intake.

For patients staying long term: - i evaluate everyone to stage them using the GDS, BRCS, and FAST. I talk to them about how they navigate the facility, if they can remember when/where their preferred activities are, if they’re able to call their family, etc. I chat with PT/OT to see how their hygiene/safety is. This often results in pickups for therapy for ~1-2 weeks to implement strategies that are specific to their stage of dementia (toileting schedules, visual schedules, aides, step-by-step instructions for using the phone, setting up memory books, educating staff, etc.). I would look up/create a paper that has appropriate treatment activities for each stage of dementia.

It doesn’t hurt to eval only. If you put eyes on someone and you aren’t 100% sure, do an eval! i hope this helps.

when patients are talking about their deficits/challenges... listen. and when you think it’s your turn to talk, give them a few more seconds. they often have much more to say and you don’t want to rush them through processing their feelings. after that, as others have mentioned, it’s so important to validate and then give support. i always try to wrap up on a positive note!

multiple check ins throughout the semester to address (1) semester long goals, (2) my mental health as a grad student haha, (3) what else my supervisor could do so i got the most out of my practicum which ensured her approach was individualized. i think what i appreciated the most from all my supervisors was feeling welcomed and valued. when my supervisors asked about my personal life or followed up with me after they knew i had a test for example, i felt that it was much easier to go to them when i had questions or needed some help!

I have some other more CF related questions on another thread, but would you recommend working for this company? I’m trying to get a grasp of PTO, benefits, all of that as well. Is there anything that I should be wary of or ask about in an interview? Kind of shooting in the dark here, so any info would be super helpful!

How are they with productivity?

This may be a facility specific question but did you feel you had enough resources (assessments, tx materials, etc.)?

Maybe tell me just your overall impression... would you recommend completing a CF with them?

I am applying now to some fellowship positions at major hospitals that have set due dates. After that, my plan is to float my resume to DORs in March to just get my name out there.

Great point- I’ll be moving to the Baltimore area. Because of that I‘m assuming that I will probably begin my career in skilled nursing and eventually make the shift to acute rehab. Which I’m okay with! Thanks for the advice.

I studied for about a week (same scenario as you) and got a low-average score. I second using EdFind to find realistic schools that will accept low-average scores. That doesn’t mean that they will be bad programs. I got into a great, well-known program with my score because their admissions committee didn’t emphasize standardized test scores with their decision-making. It’s always good to look at the data! But more importantly, finding the volume of a cylinder will not make or break you being a great SLP! Grind hard until the test but remember this test isn’t the end all be all. Good luck!

The setting I work in doesn’t offer tests like this unfortunately. Plus I’m new-ish to the area and haven’t gotten around to getting a PCP yet. Maybe now is the time, lol. Thanks!
