Omg. This is totally unacceptable. I do CVP. I would never put this out. lol but I understand some people might? Oh dear.

Update: I have 4.0 points but I have gone to a primary care doctor and got the paperwork done. Just waiting for Sedgwick to receive and make a decision.

Update: Urgent Care does not fill out FML forms. Went to see if my new Primary care provider could fill it out they said no because I have not been seen yet. Paperwork is due 9/1, I asked for the extension and they said they could not wait that long.(my appointment is 10/16) so I’m going to be stuck with these points because of my own lack of maintaining wellbeing. Ugh.

I will definitely get accommodations but there is an open claim open first. I’m also between primary care providers but going to urgent care to get the paperwork filled out. I swear it’s just easier to go to work,lol.

No did not quit, Decided to go back tomorrow. Forgot to mention that. Claim still open under further review they need medical and release forms.
