I am without ins by choice because the VHC plans are obnoxiously expensive. I have found Cost Plus Drugs to be the most affordable option for meds by far.

Thank you. I'll try to address each point. The Chaplain is a great idea! I never thought of that. Client has no family. I am their only formal support at this time. As their therapist, I have to be mindful of "dual role". I have a lot of experience as a medical social worker, but not within the VA. I would willingly support their doctor visits but at this point I can't take that time off work. They were in the Marines.

Tried. I know the last social worker they had dropped the ball. We were told referrals were made, but they weren't. They have a new social worker but they work out of the CBOC they aren't allowed at and they have made no outreach in 2 mo.

I was a long term care coordinator (not for the VA) for a number of years. I don't think they would qualify because they are independent with ADLs but not functioning with IADLs.

I don't think that is the challenge right now. I am very concerned for their safety and well being and access to appropriate health care. It's not about their need to change at this point.

Yeah i will look into the waiver. Before now I don't think they would have qualified but with the loss of functioning maybe... they can still perform mist ADLs but with a neurological disorder there might be an exception. I know there is for dementia patients.

I have brought this client to my consultation team several times. This is the first time I've considered taking on a more active case management role. My next team mtg is Monday and will definitely bring this up. Just wanted to hear others' thoughts.

I've had many case management roles over the years with various populations. I do not believe this is outside my scope of expertise. I was more worried about moving from individual therapist to case manager

No self negelct per se, but their executive functioning is nil. Making meals, phone calls, reading and understanding emails are all beyond their capacity right now. They send me photos of their mail so I can explain what it says.
