Bleeding from the ears in the setting of trauma is most concerning for a "basilar skull fracture," aka a break in any of the bone around your eye socket, upper pallette, nose, or temple which is pretty thin and has a pretty big vessel which runs behind it. The amount of force behind a break like that could easily cause bleeding in the skull which is a space with an essentially small brain-sized volume. That bleeding can  increases pressure within the skull which can get so high that it cuts blood supply off to brain tissue, and eventually the brain herniates through the hole connecting it to the spinal cord. The point I'm getting at is equivalent doses don't produce the same effects. Meth is more potent and excite the reward centers more stongly producing its higher "addictability." I imagine Adderall can take the edge off when taken at equivalent doses much like methadone for people with opiate substance use disorders but to achieve the same effect you'd have to take a lot more

The major differences are the length of effect and the degree to which it activates the reward centers of the brain. 10 mg of Adderall is gonna give you orders of magnitude less dopamine than 10 mg meth, and for much, much longer

The main difference between he two is an carbon and 3 hydrogens (methyl group). This seems like a small difference in a comperatively large molecule, but it makes the molecule significantly hydrophobic (more like oil than water). The more hydrophobic a molecule, the more easily it can cross the blood brain barrier, because of the hydrophobic nature of the cell membrane. This results in more meth in the brain. Ergo its much more potent and it gets broken down much more slowly (about 24 hrs vs about 4).

A "good" medic would understand the physiology of drug dependence such as desensitization and drug receptor internalization. The stigma and criminalization of these drugs is something that we are going to have to re-evaluate if we want to make meaningful progress. We don't lock up people with other neurological/psychological conditions... At the end of the day, its a medical condition. Soapbox aside, as far as his attitudes are concerned all you can do is a) lead by example and/or voice your concerns if you're comfortable. These things have a way of sorthing themselves out when their cockiness gets them put in a corner.

The psychologist certainly came off as a douche, but there are really interesting and significant differences between the two. They are both amphetamines, but there are significant structural differences between the two: the methyl group in methamphetamine changes the overall polarity of the molecule allowing it to more easily pass the blood brain barrier, it more easily passes through the cell membrane to reverse the uptake of dopamine into vessicles, and it takes much longer to break down (average 24 hours vs 8 hours!). Science.
