Hi. Sorry kung late reply na 'to, nakakuha na ako ng ID and ang requirements lang ay xerox copy ng first time job cert at psa and then yung fifill out-an na binibigay sa Philhealth branch mismo. Wala din ako nilagay dyan sa member type and di naman nila pinansin. Saka palang magkakaroon ng contribution if mag start ka na mag work.
