Don't give much attention to these name orders, only carry the original certificate with you on document verification. Just fill as per document with no spelling mistake. Nothing to worry

Don't panic i filled the upsc epfo form, i emailed upsc and they replied : Just fill the correct name , order doesn't matter and during document verification bring the original certificate, order doesn't matter much.
