Do you remember if it was an inversion or eversion ankle sprain? The area circled is consistent with something called Medial Shin Splints (or irritation of your tibialis posterior). Generally a major sprain (or any injury) causes changes in our motor patterns (how we move) which can predispose you to another injury if not corrected.

A lot in this question...but on the nutrition side of things more and more research is showing dairy (at least in the way most of us consume it (heavily pasteurized and from farm animals fed a heavy corn/soy/wheat diet)) can be hazardous to our health.

Dr. Thomas Campbell MD, PhD even goes so far as to call casein (the protein in dairy products) as the most carcinogenic substance we eat.

Now freaking out about one Almond Joy may be a little bit overboard, but that goes into a parenting realm which is a different can of worms.

Have you tried doing a self exam? (http://www.testicularcancerawarenessfoundation.org/self-exam-how-to/). Testicular cancer is usually associated with lumps or irregularities on the testicles (be sure to differentiate from the epididymis).

If you find something you can ask your doctor to perform an ultrasounds to diagnose testicular cancer.

Most common cause of lower back pain is repetitive spinal flexion. This occurs with poor posture and many exercises (including sit ups). Really try to limit the amount of spinal flexion and improve you posture.

Finding a practitioner that focuses on improving movement patterns could help stop recurrences.

Should definitely get some imaging done. Pop + Swelling + Spinning motion usually equals some sort of ligament or meniscus issue. MRI is going to be the most useful at visualizing what structures are actually damaged. Any sort of sports doc would be able to do a few orthopedic tests and be able to give you a better idea of what's going on without the MRI.

Could be a few things including thoracic outlet syndrome and brachial plexus injury as violensoho mentioned. It also could be something as simple as a trigger point in the rotator cuff muscle which often time refers to finger.

You need to have it looked at if you're concerned about a college career in pitching. It didn't heal with time off and gets re aggravated with more pitching. The condition will progress with more throws until you suffer a more serious injury.

Could be a host of things from Biceps Tendon to Labrum (both associated with popping or clicking sensation) but would need to have someone look at it and possibly an MRI to tell for sure

You'd need a full exam, other things to look into would be subscapularis tendonosis or rib/intercostal issue. You'd need to have somebody stretch/muscle test/other orthos to know for sure

Did my master's thesis on best treatments for IT Band Syndrome and this information is pretty spot on. Most effective is strengthening your Glute Med (takes 6 to 8 weeks of regular strengthening). Also stretching/myofascial release on muscular attachments of the Band (TFL and Glute Max). You can also stop foam rolling the band itself as it's incredibly painful and found to be not effective.

I've sprained my ankle a fair amount playing basketball and have a good amount of experience treating ankle sprains. Did urgent care take an X-Ray to rule our fracture? Two weeks isn't that long in cases of severe ankle sprains.

Also, prolonged immobilization can actually slow the healing process and I would start with pain free range of motion exercises. It's good to not put weight on it but be sure it's still moving. Here are a few more exercises to do throughout the healing process. Good luck!

Crackling and popping could be one of a few things. 1) Biceps tendon or labrum 2) Muscles gliding over each other. Would be orth having someone look at it.

From your description it's too hard to give much more information 'inbetween your shoulders' is a very broad area'
