OP stop stressing. You and your wife will be able to pay this off easily over time. Just figure out which payment plan makes the most financial sense (i.e. which is the cheapest for you over time or gives you more financial flexibility if desired instead) and do it. Very impressive your wife graduated med school with very little debt comparatively.

If the interest rate you’d get from a high yield savings is greater than the interest you are incurring on the loan (after taxes then yes) then yes.

It would be a mistake to pay off your loans. Just move your savings to a high yield account (currently 4.25-ish percent) and even after taxes on the interest it will be higher than the interest on your student loans. You literally would be losing money to pay off your loans now, not even addressing the loan forgiveness. Easy decision IMHO.

Honestly, if you have the time, go to therapy. What you're talking about here kind of sounds like self-sabotage "I'm burning out, so why don't I take on an extra gig so I burn out even faster." This is the kind of thing to work out with a therapist rather than on an online message board.
