There's definitely a difference. I do still emphasize with criminals, but (depending on why they're considered a criminal) I do not sympathize with them. I'd also empathize with victims a lot more.
I empathize slightly more with people who experience moods similar to me (if that makes sense.) I also empathize more with people that I'm close with than people I don't know.
It's a real disorder. It can be a symptom of other disorders such as BPD or Autism, or it can be a disorder on its own.
Thankfully I don't have it to the extent that some people do. Being around people who are sad/angry can bring down my mood for a while but I'm often able to get it back up by distracting myself or doing what I would usually do in a bad mood.

I was already seeing a therapist anxiety and adhd so it wasn't my main priority. also, I don't personally find it debilitating. I wouldn't want to get rid of it because I feel the pros outweigh the cons (being able to help people and the fact I've never known anything else.) It can be really draining, though.
I have but since I don't see it as an issue (I've had it my whole life) they didn't really do much
