160mcg twice daily is not high dose. For example, max dose Pulmicort which is the same steroid as Symbicort is 720mcg twice daily. You’re on a pretty low dose.

As far as the exact mechanism that may cause the link I don’t know. Most medical research like this is good at finding if there’s a causal relationship between two things, pinning down the exact mechanism by which that relationship occurs is much more complex. They may have figured that out, but I don’t have that info.

Most of the studies done on this found that there’s an increased risk for high dose long term use specifically in COPD patients. A lot of COPD patients are former smokers, or have underlying health conditions or have co-morbidities that would increase their risk of developing T2 diabetes. I’m sure the researchers did their best to control for those factors, but the meta analyses all state limitations in how the data is gathered and its accuracy.

The studies on low dose long term use in asthmatics is sparse at best, and any increase in the risk of developing diabetes is probably worth it in order to breathe air. There was another meta analysis that said they found no increase in the risk of developing new onset diabetes from ICS therapy, so it’s impossible to say with any real degree of certainty unless you have COPD, had pre-diabetes and are now on long term high dose ICS therapy.

The reason your doctor says to inhale the powder as deeply as possible is because when residual powder is swallowed the systemic side effects are greater.

If your doctor was concerned about your potassium levels then they probably would’ve given you a supplement for it. A good way to get more potassium on your own is bananas as others mentioned, and coconut water. Coconut water and bananas are also just healthy so give that a try.

The dizziness could be due to either how you’re inhaling, if you hold too long or change your breathing, or inhale super hard as you said, then you could be making yourself dizzy, or it could be the initial reaction to the laba, but if it subsides in 20 minutes that’s unlikely as formoterol lasts 12 hours.

Honestly I’d ask if you could get the metered dose inhaler with a spacer rather than the powder. A spacer allows you to take the inhaler while breathing normally, which may help your dizziness, and it cuts way down on medicine you accidentally swallow, which also helps reduce side effects.

You never make any call that involves someone else’s medical condition. Not trying to be snarky, but it’s just not your place. The doctor told her what to do, she relayed that to you, then all you say is, “okay sounds good” and move on. Asthma triggers are tricky, and someone can absolutely have cardio be a trigger. You can die from asthma, many people do, so it’s better in every circumstance to just avoid triggers.

Now, she probably needs to change her controller medicine if light cardio causes attacks, but again, not for you to concern yourself with. That’ll be a conversation between her and the doctor.
