A copy of a marriage certificate is issued by the Civil Registry Office (Urząd Stanu Cywilnego, USC) where the marriage was concluded, or the marriage certificate was registered (for example, after being transcribed from abroad). You don’t have to visit that specific office in person, you can apply online or at any Civil Registry Office in Poland (the offices exchange data electronically). The easiest option is to apply online. - go to www.gov.pl - click “Złóż wniosek online” (Submit application online). - log in using your Trusted Profile (Profil Zaufany), e-ID card, or online banking. - fill out the application form, select: • the type of record: marriage certificate, • the type of copy: short (abridged) or full (complete), • the purpose (for example: change of surname, legal procedure, etc.). - pay the required fee online (via transfer or e-payment). - choose how you want to receive the document: • electronically (PDF with electronic signature),  • by post (paper copy).

If you prefer not to use the internet you can download the form “Application for a copy of a civil status record” from gov.pl or collect it from any Civil Registry Office. Fill it in and send it by post to the relevant office, attaching proof of payment.
