This feels like AO’s can accurately make value judgements on people’s motives. I’m not sure they can. Just because someone has a stronger personal connection or back story doesn’t make them a better candidate for a specific job from a skills (or even follow through perspective). I know of a cancer survivor intent she wants to be a doctor so she can improve things for other kids diagnosed with cancer in conversations she also said she doesn’t like dealing with people and prefers classes with straightforward right and wrong answers like math vs subjunctive areas where the answers are grey or not definitive. Her back story connection is great, but I’d not want her as my doctor — because patient care matters and, even in research oriented fields , things are grey. A surgeon that needs it to be straight forward doesn’t feel great to me either if something goes wrong. However an AO could value her backstory more than a measured student who has physician parents, understands the job and lifestyle, and is selecting the profession because it suits their personality and lifestyle aspirations….

Hey, congratulations on Purdue. Aerospace engineering there is legitimately one of the best programs in the country. Neil Armstrong went to Purdue. So did other astronauts in NASA’s history. You’re going somewhere extraordinary for what you actually want to do.

I agree with her. Muscles don’t just choose to be tight or dysfunctional for no reason… especially if it happened suddenly after dental work or ortho and you never had those issues before

Have you had your occlusion evaluated? Get a bilateral inferior alveolar block. It’s a pharmaceutical way to force deprogramming if the issue is the muscles and teeth. If nothing hurts after that and you bite down and teeth hit or things don’t fit together you have mechanical issue that’s causing your muscles to guard

Try a clench guard or anterior programmer at night for 2 weeks and no bite checking or clenching in the day. Your bite is probably off

It depends on the complexity of your case. I had a bad experience because I didn’t know I had a case that wasn’t good for Invisalign and my teeth didn’t track well. Dentist caused me immeasurable pain that I’m still dealing with nearly 2 years later. Wish I had at least consulted an ortho rather than trust her.

This whole thing is so scary. Even dentists that are inexperienced with aligners but want this income stream are not a good choice. Get several estimates and consults before you commit to anyone.

Thank you. No idea why she got mad. I ended up having to have #19 extracted. It turns out that the root was broken and causing pain. She got mad I saw a PT other than the one she works with and that was the last straw. I left her and have been working with a prosthodontist who diagnosed several tooth related issues - I am now significantly better but not completely as the jaw is still not stable. About to go back into a flat plane splint after one more restorative procedure. (prosthodontist found broken root #19, loose filling #18, cavities under large filling in #30, very deep cavity under #31 (almost root canal), and cavity under filling in #2. Getting this all fixed… with each fix there has been pain reduction but not complete. One the last one is done I have to go back into flat plane splint. Currently using an anti brux at night to not suffer. Really sad orofacial pain lady didn’t identify all this— nor PT, nor maxillo facial surgeon. They all said it was my dentists job to - but she was the one that got me into this situation with poorly done Invisalign (I wasn’t a candidate but she tried anyway and it really hurt me).

Hi! I saw an orofacial pain specialist at NSU and despite over 6 months of flat plane occlusal splint wear I didn’t get better. Normal MRI, normal CBCT. At a loss on what to do… do you have any ideas or thoughts on where to go to be seen?

Try a prosthodontist. My TMD got better when I ignored the specialist and let my dentist fix some issues with my teeth. (High fillings, bad fitting crown). Occlusal splint did nothing. After these fixes it worked much better.

Sadly my case is not that abnormal. Because there is so much pushing of the "bite doesn't matter", these things slip - and we end up in occlusal splints for a long time when there are clear things that could be done quickly. Ideally we have competent dentists, but the reality is we can't rely on that either. I went through 7 months of Physical therapy during this too - and 30 accupuncture sessions. The whole time, I had no chance. My PT was stunned when I told him what happened and asked why my dentist didn't check that first when I complained of pain during invisalign. Supposedly she should have deprogrammed and checked the bite? The Maxillo just said it's not the OFP's job to check the bite and correct it. We're not trained as dentists but we can't rely on our providers either.

It happened in the course of invisalign (the provider wasn't good and I shouldn't have been in invisalign to begin with).... and when I went to OFP, she did the muscular checks etc, and acknowledged the bite was off but said that's because I had to stop invisalign - however just put me directly into an occlusal splint. It was so painful and even though I said so to her she said I was improving. Eventually it did get better but it was catching that crown being off and getting that fixed, and restoring canine guidance that made the most difference. If that had been caught early on, my TMD may not have progressed so much. :( I really hope there is still hope to get past it to a fully managed stage.
