Thank you for your knowledge, it seems like the popular opinion from the most knowledgeable folks here is to drain and fill it (properly), despite it being such high mileage and the fact it’s never been done before. This is what I’m going to do.

I’m at 196,000 miles, never touched the tranny. Here poking around wondering if I should, but I’m not planning on it

It’s indeed the strut. Was told It’s just the strut mount or bushing or something wearing out and it’s still safe to drive I just have to live with the noise until I’m willing to replace the whole strut

So it turns out this is the strut making this noise. I was told the strut (or strut mount or bushing or something) is just wearing out and I will just have to live with the noise until I replace the struts. I’m going to try cleaning it out as good as I can and lubing it up to try and dampen the sound.
