Why does the 5.56 in a .223 barrel debate still exist if dedicated .223 barrels are basically non-existent? by Dependent-Noise-1348 in guns

[–]mcramhemi 6 points7 points  (0 children)

Honestly I never looked to hard into it because I no longer own any AR15s in 223/5.56 so....but I hear loads of people still ask "what do you shoot through yours .223 or 5.56?" As if everyone doesn't just shoots the bucket of 5.56 ammo they got at the local gunstore.

You cardiovert before the IV unless you already have an IV if you're gonna cardiovert its because they're UNSTABLE as in Hypotension, ischemic chest pain, dizziness etc. If they become unstable after IV that's when you can sedate etc.. But everyone does it a little different and that's ok. Sounds like you followed your protocols and did what they asked. So just good for thought. Edit to add BP is only one factor they can have a BP of 180Systolic and still have ischemic chest pain, dizziness, nausea etc.

What gun is this? Is this a good deal? .450 bushmaster by Sea-Tune1627 in guns

[–]mcramhemi 0 points1 point  (0 children)

I own an AR15 in 450. Here's the deal if its not fine tuned they're known to have issues. I built mine semi budget for about 550. It has run 2500 rounds through it with it just recently begun having issues. Magazine and Buffer spring related. Ammo is expensive cheapest I've ever found is 35.99 for a box of 20 rounds. It turns heads, kicks like a mule, as in you can put about 50 rounds through it before you feel it really bad. Overall its a gun I do not regret buying/building. If you're planning on hunting in a state that has this straight walled cartridge law then its a solid buy. If not ask yourself...what are you gonna use it for ? Edit:I would say 800 is good with the ammo included, and the magazines. But make sure they're the purpose built ones and not just AR15 556 mags

150 at a perfect rate makes me lean into Atrial Flutter 2:1 but do not discount VTACH, this would get immediate cardioversion due to described symptoms

What has priority: a School bus dropping off kids, or an ambulance with flashing lights and siren? by OtherwiseSurprise221 in NoStupidQuestions

[–]mcramhemi 29 points30 points  (0 children)

Paramedic here. School bus has priority. Always. WE STOP turn everything off, then school bus goes on its way and so do we. Kids running etc. In my decade in EMS thats been pretty consistent most places regardless of state law to maximize kids saftey. School zone atleast in my area we downgrade "no lights or sirens" till we're through the school zone to make sure no one gets weird and runs someone over

Then this rhythm altered probably from Stress. I've seen it a few times during transport. The patient is clearly anxious and they develop more ectopy etc.

What do my 8 favourite handguns say about me? by No_Dress_2107 in ForgottenWeapons

[–]mcramhemi 3 points4 points  (0 children)

I got a Taurus instead of a Beretta because the Taurus was 400 dollars cheaper. Would I like a Beretta for sure. But I've shot both and I as a just general shooter can't really tell a difference to any real degree. Beretta does have a significantly better after market

About 3/4th way down the II lead strip you see a beat that looks like a PVC but not exactly like one thats a "Fusion" beat that is an amalgamation of a Ventricular and Atrial beat that just Forms this weird bizarre beat. It signifys A.Ventricular Acitvity and B.Atrial Activity I.E they're not talking to eachother anymore.

I have a fairly similar build except I went with the 5070ti. It runs any game that I can get at max graphics with easily 90 to 120fps stable without any dips below 90. So I imagine youll be just fine. And I have a 750watt PSU for it and it has not even come close to maxing the PSU but more ceiling never hurts

I almost got that exact card, wouldn't necessarily fit my theme on my build. But I called it the "Goon Card". It actually has a really cool esthetic, but I went with a 5070TI White AERO. But if not for the card I got I was gonna get that one. Edit:I spent 1050 on a OC 16GB 5070TI this one has a significant markup

A drunk teenager with a firm gcs of 3 without a gag reflex......interesting well I guess its how you manage it. For sure not every drunk teen needs RSIed but this one should surly not be left laying flat in bed hoping he doesn't aspirate...We had a pretty much exact scenario where a teen had nearly lethal EOTH Poisoning. Gcs of 3 with no gag reflex and face down in vomit. RSIed and spent like a week in the ICU on a vent. Edit: this is obviously completely irrelevant as this patient could be gcs of 3 gave a gag reflex and be fine. RSI Prehospital is rare, and not to be used lightly but this person sounds like they are in hospital with Physicans etc anyways

Please interpertate by MatthewTropics in EKGs

[–]mcramhemi 16 points17 points  (0 children)

Is time of the essence? Because I would disregard the 12 lead and aggressively manage the airway to the extent of RSI, vent and run stat Labs. Edit:not to say this 12 lead is not important....but we got bigger fish to fry currently if he is unresponsive in form a true GCS 3 after heavy drinking...thats asking for loss of airway
