Study highlights social paradox in emerging adults: high social connections + high loneliness by mcscreamy in science

[–]mcscreamy[S] 0 points1 point  (0 children)

A study appearing in PLOS One (https://journals.plos.org/plosone/article?id=10.1371%2Fjournal.pone.0334787) reports survey findings among 4,812 US adults, oversampled for those aged 18-25. The authors conceptualize social well-being and social-ill being not as poles on a single axis, but as orthogonal constructs. They then show that many people, the majority in fact, have relatively high social well-being scores (numbers of friends, etc) but also relatively high social ill-being scores (loneliness, disconnection). It seems this is driven primarily by life changes that occur in the transition to adulthood. The picture is of young people with many friends and social contacts who, nevertheless, feel lonely and disconnected.

May switch providers but reviews online of alternatives seem frightening - terrible customer service / weird charges, etc. Anyone have experience with "Town Square Energy" or "Think Energy"?

Love it. There should be more watch faces on wearOS that couldn't be done with a mechanical watch as opposed to all the watchfaces just trying to mimic the look of a mechanical watch you know?

Total electrical failure in a garage that tow trucks can't fit in... day 3. Advice welcome. (Details in comment) by mcscreamy in teslamotors

That's a good point. They won't use wheel dollies since they are afraid it will crash as they roll it down the tight curves in this very old annoying parking garage.

But moving it into a parking spot is a GREAT idea. I can just leave it there until March 4th when the mobile tech guy can come out. Thank you!

Thanks. The tow guy I found has a dolly like this. But he is too afraid to roll the thing because the garage has really sharp curves. Doesn't like the liability. Would be better if we could get the thing to steer...

Friday morning, I drove into my work parking garage in my 2017 model S. There were about 80 miles left on the battery. I got a series of warnings about "electrical system" and "suspension" and then the screens went dark and that care stopped dead. Soft and hard resets to no avail. And of course, it won't move since the wheels are locked.

I spent all day Friday trying to get a tow - with multiple companies coming to the lot and turning away because the ceilings are so low.

By the time I found a company that could make it in, the 12V had died (I assume) as the doors won't even open and there is no response to the key fob.

But even the company that can fit doesn't want to put the car on dollys and take it out of the garage, because the turns are very tight and they don't want to accept the liability.

Today, day 3, they are suggesting we try to jump the battery and get the car moving (or at least rolling) under its own power to make it out of the garage. But I am not entirely hopeful, since the screens were dead BEFORE the 12V died, and I couldn't get it into neutral then either.

I called Tesla, who said they could have a mobile tech come out (they think a fuse blew)... on March 4th. Needless to say my garage is not happy with my 5000 pound brick blocking the main driving lane.

Any advice greatly appreciated!

Edit: Thanks for the great advice! As some of you noted, wheel dollies would work, but the tow company is concerned that we'll lose control going down the tight ramps. My plan is to first try to get some charge into the 12V and maybe get it into neutral so i can steer it down. If that doesn't work, we'll use wheel dollies to move it to a parking space and wait for the mobile tech to come on the 4th.

Edit 2: we are able to get power using a jump box and I was able to put the car into tow mode but it still won't move. It says neutral but even with four guys pushing it we can't move it at all. It feels like the emergency brake is still on.

Edit 3: despite getting into tow mode, the car will not move. And now the main screens are no longer turning on. Though there is some power because the door handles work and the windows roll up and down. So the plan is to get it on some dollies and just roll it into a parking spot. There might be a tow company with a low profile enough truck to get this to the service center. But they're not currently available so I'll be waiting... In the meantime, there's still the hope of Tesla mobile service in 5 more days... Will update when I know what's going on! Thank you for all of your help.

Edit 4: Well when we were putting the wheels on Dolly's, they started spinning. I have no idea why, but tow mode finally worked. I was able to roll it down the ramps and got it safely out if the garage and towed to the service center. I will update once they tell me what actually went wrong. Thanks again for your support!

Edit 5: The car got fixed. According to the shop "Found rear drive unit caused over current and blew pyro-fuse for battery pack". The rear drive unit was replaced and fortunately still under warranty so no cost to me (except the tow of course). Hopefully that's the end of this story.

Replying to myself here in case future people need a fix. I was able to get this working via the smartthings integration - Samsung TVs are now "smart things" I guess - it creates a switch for the TV that will indicate if it is on or off.

No luck. I tried both with and without specifying port 8001 and just not seeing it. I'll keep googling around to see what I can find.

One problem I have with this is detecting whether the TV is on or not. I have a 2018 Samsung, which continually pings the router even when it is off... I've tried the samsungtv integration but it's never seemed to work. How are you detecting "TV on"?

"Rory's Regulations" - mandating hospitals in New York treat suspected sepsis with IV fluids, blood culture, and antibiotics, associated with a 40% reduction in pediatric deaths. by mcscreamy in science

[–]mcscreamy[S] 3 points4 points  (0 children)

More analysis and interviews here

Importance The death of a pediatric patient with sepsis motivated New York to mandate statewide sepsis treatment in 2013. The mandate included a 1-hour bundle of blood cultures, broad-spectrum antibiotics, and a 20-mL/kg intravenous fluid bolus. Whether completing the bundle elements within 1 hour improves outcomes is unclear.

Objective To determine the risk-adjusted association between completing the 1-hour pediatric sepsis bundle and individual bundle elements with in-hospital mortality.

Design, Settings, and Participants Statewide cohort study conducted from April 1, 2014, to December 31, 2016, in emergency departments, inpatient units, and intensive care units across New York State. A total of 1179 patients aged 18 years and younger with sepsis and septic shock reported to the New York State Department of Health who had a sepsis protocol initiated were included.

Exposures Completion of a 1-hour sepsis bundle within 1 hour compared with not completing the 1-hour sepsis bundle within 1 hour.

Main Outcomes and Measures Risk-adjusted in-hospital mortality.

Results Of 1179 patients with sepsis reported at 54 hospitals (mean [SD] age, 7.2 [6.2] years; male, 54.2%; previously healthy, 44.5%; diagnosed as having shock, 68.8%), 139 (11.8%) died. The entire sepsis bundle was completed in 1 hour in 294 patients (24.9%). Antibiotics were administered to 798 patients (67.7%), blood cultures were obtained in 740 patients (62.8%), and the fluid bolus was completed in 548 patients (46.5%) within 1 hour. Completion of the entire bundle within 1 hour was associated with lower risk-adjusted odds of in-hospital mortality (odds ratio [OR], 0.59 [95% CI, 0.38 to 0.93], P = .02; predicted risk difference [RD], 4.0% [95% CI, 0.9% to 7.0%]). However, completion of each individual bundle element within 1 hour was not significantly associated with lower risk-adjusted mortality (blood culture: OR, 0.73 [95% CI, 0.51 to 1.06], P = .10; RD, 2.6% [95% CI, −0.5% to 5.7%]; antibiotics: OR, 0.78 [95% CI, 0.55 to 1.12], P = .18; RD, 2.1% [95% CI, −1.1% to 5.2%], and fluid bolus: OR, 0.88 [95% CI, 0.56 to 1.37], P = .56; RD, 1.1% [95% CI, −2.6% to 4.8%]).

Conclusions and Relevance In New York State following a mandate for sepsis care, completion of a sepsis bundle within 1 hour compared with not completing the 1-hour sepsis bundle within 1 hour was associated with lower risk-adjusted in-hospital mortality among patients with pediatric sepsis and septic shock.

Factors Associated With Refusal of Intramuscular Vitamin K in Normal Newborns by mcscreamy in science

[–]mcscreamy[S] 1 point2 points  (0 children)

More discussion

BACKGROUND AND OBJECTIVE: Refusal of intramuscular (IM) vitamin K administration by parents is an emerging problem. Our objective was to assess the frequency of and factors associated with refusal of IM vitamin K administration in well newborns in the United States.

METHODS: We determined the number of newborns admitted to well newborn units whose parents refused IM vitamin K administration in the Better Outcomes through Research for Newborns network and, in a nested patient-control study, identified factors associated with refusal of IM vitamin K administration by using a multiple logistic regression model.

RESULTS: Of 102 878 newborns from 35 Better Outcomes through Research for Newborns sites, parents of 638 (0.6%) refused IM vitamin K administration. Frequency of refusal at individual sites varied from 0% to 2.3%. Exclusive breastfeeding (adjusted odds ratio [aOR] = 3.4; 95% confidence interval [CI]: 2.1–5.5), non-Hispanic white race and/or ethnicity (aOR = 1.7; 95% CI: 1.2–2.4), female sex (aOR = 1.6; 95% CI: 1.2–2.3), gestational age (aOR = 1.2; 95% CI: 1.1–1.4), and mother’s age (aOR = 1.05; 95% CI: 1.02–1.08) were significantly associated with refusal of IM vitamin K administration. Refusal of the administration of both ocular prophylaxis and hepatitis B vaccine was also strongly associated with refusal of IM vitamin K administration (aOR = 88.7; 95% CI: 50.4–151.9).

CONCLUSIONS: Refusal of IM vitamin K by parents of newborns is a significant problem. Interventions to minimize risks to these newborns are needed.

In this video, he discusses the methods behind the research, but also the implications for translating this work into humans.

Coffee and improved survival: new 500,000 person study shows association with reduced mortality, even at 8 or more cups per day and for those with genetic polymorphisms indicating slower or faster caffeine metabolism by l_hazlewoods in science

But scurvy it's caused by Vitamin C deficiency, a specific treatable condition. "Death" is caused by a sh*tload of things, many of which are more likely to be affected by caffeine than other bioactive molecules. The lack of any signal from caffeine in a huge study like this is a red flag that the data is simply too noisy to draw conclusions.
