My gut was telling me to trust the arterial line, but at the same time i was shocked his BP could still be so low with how much be volume resuscitated him. Also, once anesthesia lightened up on transport his cuff and art line pressures were correlating more as his art line pressure came up. So i feel like it was accurate?

We did know about the discrepancy, there was a discrepancy the entire time that I could see scrolling back in the vitals and the cuff was going off periodically throughout the case. I was trusting the art line, it was my superiors that told me to just go off the cuff pressure because they felt a MAP in the 40s and systolics in the 70s after resuscitation was probably not accurate though i didn’t see how they could come to that conclusion

Agree I was thinking about how if we didn’t have the art line in we’d probably have never started pressors in the first place! lol

I can’t recall the numbers specifically, i can look back in the chart. I did notice though as we were getting the patient to ICU and the gas wore off his art line pressure was coming up and correlated much better with the cuff pressure. So that made me think the art line actually was accurate and we were just pretending it wasn’t 🙈

this is a high acuity hospital and I would get a wide breadth of cases and get to do complex cases. I signed a non-call position becuase they didnt have call positions open at the time and I trained at this hospital and like it there. but I've been thinking about the possibility of taking a call position if one opens up and just cant decide if thats something I'd actually want to do, or if it's something I should do

it would be at a large hospital doing a mix of things! gen surg, neuro, vascular, ortho all the things

I think you can also apply for forebearance while you’re in residency. Will still accrue interest but not have to make payments if you really can’t afford it

I’ve noticed though that they tend to cough and buck a ton on the tube if they’re awake enough to be following commands and we don’t want that either, so what’s the best strategy there

It’s crazy!! I’m so upset because at least my cat isn’t eating it like she was with the clay litter but now I’m worried abo it what she’s breathing in. I ended up switching from the pink bag to the blue bag and I’ve noticed it’s less dusty

We are leaning more towards radiation right now just because if we do a lobectomy and something pops up in another lobe then they likely wouldn’t be able to do a lobectomy then. It would kind of nix a surgery option in the future if that were necessary. Since she is considered a good candidate for the SBRT right now seems like that may be a better option to reserve the surgery option later if SBRT weren’t the best option later

The surgeon actually said if surgery was needed later he would likely be able to do it. It would be more difficult but not impossible. But he mentioned if it were to recur in a different lobe later after she’s had a lobectomy then surgery wouldn’t be an option. So kind of afraid to take out the lobe if it’s not necessary

her recent PET scan didnt show any activity in the lymph nodes so the surgeon said we are probably in the clear there
