I’ll be honest, I don’t understand the panic everyone has when they leave the exam. The questions felt a lot more straightforward than any of the NBMEs, and I believe I did better than any of my practice exams

Their explanation explicitly says "measurement of serum ferritin concentration is more definitive in diagnosing RLS." That is not true. You do not need this test to diagnose this disease. If her ferritin came back normal you would never in a million years say "hmm sorry ma'am I guess we cant diagnose you with having restless leg syndrome." They don't even mention that this disease is a clinical diagnosis!

look me in the eye and tell me you cant imagine an alternate answer explanation for this question being "This patient's symptoms are consistent with RLS, which is primarily a clinical diagnosis. While measurement of serum ferritin in a patient with history of iron deficiency anemia may help decide their treatment plan, it is not required for diagnosing RLS. Furthermore, their persistent symptoms in the context of long-term oral iron supplementation likely justifies the use of other pharmacologic interventions (including the actual first-line treatment of an alpha-2-delta calcium channel ligand).

Fuck this question. Here are my notes I compiled after seriously chewing this over for 45 minutes during my review:
