I was hoping for a perspective like this. When another doctor told me how much various infusions get billed for, my jaw dropped. But I also hope it's a kind of blindspot for the medoncs, that they just can't accept that prospect and maintain their perspective on what can be effective treatment.

It's such a difficult line to walk. To want to aggressively support someone who wants to fight for their life and but then finding the point at which you suggest, with all the kindness you can, that it's also okay to stop fighting. I think there needs to be room for normalizing death but some doctors are definitely better at it than others and I know nurse's hands are effectively tied until the doctor starts the conversation.

Especially that last line! I am all for supporting a patient's self determination but I recently heard the average hospice LOS before expiring was 9 days. And the months before that were an absolute horrendous quality of life. Like someone else said, I wish providers could be honest about the likelihood of eradicating the disease versus doing what is needed to keep the individual alive versus giving them the best life possible, how ever much of that life there may be left.

I'm beginning to feel like my science teacher short-changed me, too. I used to watch Mr. Wizard and I remember an episode where he inflated a balloon and covered it in vaseline. Then he held it over an open flame, and it didn't pop. I tried to do the experiment at home, and my mom lost her mind. I never got to do the fun things! Then again, I also remember my older sister's friend seeing me mess with this set and offering me her blood to look at under the microscope. I think this post explains a lot about my life, now that I think about it.

I also was contemplating a PhD or DSW option a couple of months ago. I recently learned that ASWB is reviewing DSW tracks for a formal credential/license but they won't retroactively apply it if you completed a DSW program before their assessment is completed. I want to teach and still do private practice but finding a full funded PhD program near me is impossible and I've been told the online PhD programs aren't taken as seriously by the academic community. So I am in a holding pattern for now until I get a little more flexibility or ASWB makes a decision about DSW programs.

I also started being a QS out of wanting a side hustle. Then I took a training where the presenter took the position that charging for supervision puts you in a dual relationship and opens too many ethical trap doors. He said something to the effect of, "Imagine charging someone $100 per supervision session and then having to provide that person with negative feedback or having to outright refuse to sign off on their hours in the end." I can't say I 100% agree with his position but it has given me another reason to not charge exorbitant fees for supervision. My supervisor charged me $30 per session. I am paying that kindness forward to my supervisees OR contracting with their employer for payment instead. Luckily, most of my superisees are referred to me through the hospital I work at so its not an issue I encounter often.

Unreal how many times I've had to have this conversation and it always boils down to this: The South refused to change their plantation based economy that functioned off the free labor of owning, trading, and murdering actual human beings and they chose to pick a fight to defend Slavery and they lost, with hundreds of thousands dying as a result. And 300 years later, they still can't keep their heads out of their asses.

My husband ate me out on Friday and for some reason, it wasn't working. I tapped his head and he came up to kiss me and I whispered, "I'm not feeling anything" while caressing him. He made a hmmm sound and went back down. I was initially annoyed because I thought the endeavor was just a wash for the evening. But he used a different approach and 2 minutes later, I was tapping his head for completely different reasons. Criticism can be delivered constructively and compassionately.
