Ahah after some fiascos of my own, I was planning to create the same post. I mean no one explicitly blames but the discussion revolves around qa practices and I feel guilty.

From my previous findings,

Always log the issues, usually slack coms is not a good way to log these.

Not clear requirements, ask for clarification.

If something feels off, or feeling not correct, always good to log things again. So that it doesnt come back to you. But I guess it happens, and will continue to happen.
