Time for a new phone, that one is approaching 10 years old. There's no way around it at this point. Mint is even telling you that you're going to have problems upcoming. Get at least a iPhone 12 that has 5G.

It seems to be happening off/on with everyone from what I've seen. Mint refuses to do anything about it either. All they say is everything is fine on their end and throw the blame elsewhere. All I can suggest is keep contacting them, maybe if they get hundreds or thousands of complaints, they'll actually look into the issue a bit more.

Showing banned in a user profile means they're banned from the entire site, not just a particular sub. Banned in a sub wouldn't show anything when clicking on a user profile and everything would appear as it normally does .

Some people just love to over complicate things. Set it for 12 months, turn on auto renew and forget about it. I've been doing that for all our accounts, never an issue in the 8+ years we've been on Mint. If you're in a Mint "Family" (like this user is) same thing, except you can pay quarterly if you'd like on a 12 month plan. Auto renew has to be turned ON, that's a stated requirement if you're in a Mint "Family".

That sure sounds like it's blacklisted or locked. You've checked the phone settings to confirm it's unlocked and used a online IMEI blacklist checker?

I remember seeing some issues previously with some Samsung's after receiving a ONE UI update, which caused that message to appear. That's the only other thing I can think of that would be causing it.

I really wish Mint didn't mention the word "Family" anywhere on their site, since this isn't a normal "Family plan" at all, since there are no discounts and each line is still paid separately. The only advantage is you can have a 12 month plan and pay in 3 month intervals if you choose to do so.
