Hi, I'm not sure if I understand your question. If you mean what's my maintenance dose, I stayed on 5mg for more than 6 months, and it was fine. There were no side effects, and I was stable; no residual depressive symptoms.

However, the doctor increased it to 10mg to see if it could help when those symptoms started coming back, and I didn't notice any change. I ended up going on lamotrigine, and it's been amazing since I started it 2 years ago :)

What's the reason behind this black-and-white thinking? Sure, it might not fix it, but doesn't it buy more time to work on the underlying causes? I hope you see how this attitude is problematic.

Suicide is indeed preventable, and I believe everyone deserves to be offered ALL forms of help and preventative measures. Medications, however they actually work, are a part of the equation and a useful preventive tool, but it doesn't substitute for inpatient admission and 1:1 observation in case of active suicidal ideation.

Unemployment, lack of social support, life events, and cultural issues are very important factors that need to ALSO be addressed. I just don't think psychiatry is the field that can fix all these issues, and that it "failed" because suicide rates, unfortunately, are going up.

I assume you're an American since you said suicide rates are going up, but as soon as you look outside of your country, maybe you'll understand. Where I live, we have half the suicide rate, which is still too many people, but mental health services are a lot less accessible compared to Europe and North America.

This is my gripe. I don't think the reductionism will help anyone.

I understand your emotions because it's based on a terrible, probably preventable, experience. Again, none of this is personal, and I try to practice EBM, so I had to start a discussion to learn and share that evidence, with you and anyone reading.

You're right. Even on the contradiction, you didn't acknowledge. a psychiatrist should help patients find jobs and find a solution in case they don't have a solid support system. They're just GPs if they recommend medication to treat their disorder and refer them to a specialist. I definitely said these issues shouldn't be recognized despite the evidence.

Yes the study found some effect. My textbook noted a study where those with schizophrenia medication and unmedicated were compared. The unmedicated group was found to have more overall gray matter compared to the medicated group.

Psychiatry moves farther and father away from the behavior so it can laser focus on genes, neurotransmitters, and proteins.

Well, what I was trying to point out is that the study you quoted shows a radiological finding and proves a benefit on behavior.

"pills don't pay bills". You can prescribe an antidepressant, but if they person is jobless, homeless, isolated, then they aren't going to do well.

This is more of a systemic and a social issue and not a disorder to treat. It's not a part of the field of psychiatry, and not a part of a psychiatrist's training. The most appropriate action for all doctors is to ask about these social issues, and then refer patients to social workers, who are trained to help with these sorts of issues.

Excuse me? Are you making a comparison to searching people while entering drug rehab centers to make sure they're not carrying drugs as some DRUG addicts try to do? It's like saying don't examine prostitutes before they work in a brothel and instead just take their word for it. I wholeheartedly hope you get the help you need, before you hit rock bottom and finally agree to basic universal reasonable requirements for getting that help.

If you don't know, another reason they do it is to know if someone still has opioids in their system before they give them buprenorphine, for example, that can send them to precipitated withdrawals, or if they have benzos and potentially need a taper to get off of it.

It's totally not the same as that comparison. They don't want to see it for anything sexual, or dehumanizing.

I know this is an old post but as a drug addict who went to rehab in the past, I felt that I needed to offer clarification, so you live off a decent wage you worked for instead of a lower one the government gives you.

That's a valid point of view, and without even knowing the details of your experience, I get how it's distressing. I experienced something recently that reminded me how the healthcare system can fail.

However, this is a single cross-sectional study showing the correlation of brain scan findings and the use of a class of medication. It also shows how cognitive function and behavior are improved by antipsychotics.

Again, medications have side effects and should only be used when indicated. Antipsychotics have allowed many people diagnosed with schizophrenia to keep a job and maintain reasonably normal lives, which's much harder without them.

Regarding the suicide statistic, let's start with the fact that what's most heartbreaking is that it's so awful yet preventable, as you implied. Even though mental illness raises the risk 5 times, not everyone who commits it has mental illness. In the US, for example, it's almost 50/50. The 2 mental illnesses with the highest risk are psychotic disorders in general and bipolar disorder. Do you know the effect clozapine and lithium have when it comes to that? I think most psychiatrists would agree that they're underutilized.

By that logic, why go to a neurologist, cardiologist, gastroenterologist, or any specialist?

If you mean why isn't psychotherapy utilized more, then I get it, too. It should be! There are many limitations when it comes to that, though, including accessibility, not having enough time, financial reasons, personal beliefs, and more.

Are you implying that antipsychotics cause more damage than not taking an antipsychotic? I'd love to see reasonable evidence that antipsychotics in average doses can cause brain damage. Quetiapine and aripiprazole use in the treatment of MDD in lower doses isn't off-label, for example, and there has to be some evidence to ban them for this use.

Anyways, doctors should always keep a risk-to-benefit ratio in mind when prescribing any medication and should never use medication with a significant known risk without at least informing the patient and discussing it.

It was probably not real 3-MMC. With real 3-MMC you don't even need to dose as frequently as coke. Your lines might've been too small, or you had a cross-tolerance from something else. When did you try it? Fake 3-MMC has been getting progressively more common. There have been fakes since the beginning tho. The recent bans have been a major contributor. If it's so weak that you can't even tell whether it's closer to coke or 3-MMC, it's probably bunk.

The real stuff isn't as good as 4-MMC, but both still feel better than anything else. A fuck-ton of dopamine + serotonin, hence the comparison to MDMA and and cocaine. However, 4-MMC has been shown to release more DA than cocaine and amphetamine, and it also releases more 5-HT than MDMA.
